利用結構式超音波圖像積分正確預測卵巢癌的探討

摘要


卵巢癌是台灣女性生殖道癌症中發生率排名第二位的惡性腫瘤,僅次於子宮頸癌。由於女性卵巢位於骨盆腔內,產生之惡性腫瘤不易早期診斷,根據衛生署的統計資料指出,台灣婦女因卵巢癌死亡之病例有逐年增加的趨勢,因此對卵巢癌的發生需有正確的認知,如能提早的發現,便可提高治療之機率。 目的: 探討超音波的圖像積分與卵巢腫瘤的患者罹患惡性卵巢癌的機率是否有明顯的關係,以便去提前預測卵巢癌發生的機率,降低其死亡率。 方法: 由中山醫學大學附屬醫院的婦產科提供相關卵巢腫瘤的患者,以人工的方式去記錄各種病理報告與血液數據,並且將患者的超音波指標(Sonography morphology index)的方法去算出圖像積分,之後利用SPSS統計軟體去探討圖像積分與各變數之間的關係。 結果: 超音波圖像積分與卵巢癌與的關係,所得的敏感度為0.778,精確度為0.939,陽性預測值為0.875,陰性預測值為0.886。陽性檢定的概述比估計值為7.773,陰性檢定的概述比估計值為0.451,另外超音波圖像積分與卵巢切除等外科手術亦達到顯著的關係。 結論: 利用超音波的圖像積分可以去提前預測卵巢腫瘤的患者罹患惡性卵巢癌的機率。所以藉由超音波的圖像積分可以在不浪費醫療資療的情形下提早抑制病情的惡化,甚至可以延長生命。

The ovarian cancer is the malignant tumor that the incidence ranked the second in cancer statistic of Taiwanese women's imalignancy, second only to the cervical cancer. Because the ovarian of women lie in the bony pelvis,that the malignant tumor is difficult for early diagnosis. There is an increasing tradeny of ovarian cancer. According to the statistical data of National Health Administration, Taiwanese women have the tendency to increase death because of the mortling. Offering the patient that relevant ovarian tumor of gynaecology and obstetrics of the affiliated by the Hospital of Chung Shan Medical University, recorded various kinds of pathology reports and blood data by manpower, and use the method of Sonography morphology index collecting all the morphology index, and used SPSS to count the software to probe into the relation between the morphology index and every parameter. Morphology index and the ovarian cancer,the Sensitivity 0.778, the Specificity is 0.939, the PPV is 0.875, the NPV is 0.886,the LR(+) is 12.75,the LR(-) is 0.236. Adopt Sonography morphology index to distingrish early the probability of the malignant ovarian cancer. Therefore, the result of morphology index can suppress the deterioration of the ovarian tumor without wasting of medical resource.
