不同樹脂黏著劑對於二矽酸鋰嵌體的抗壓強度影響

The compressive strength of lithium disilicate inlay among different resin cements

摘要


牙科陶瓷贋復物不僅兼具美觀、功能和強度,近幾年在微創牙科治療的風行下,蔚為主流。包括陶瓷貼片、陶瓷嵌體、覆體,材料方面有傳統長石陶瓷、白榴石基底、二矽酸鋰基底、二氧化鋯。 本實驗應用了微創的概念,選擇人類上顎24顆小臼齒做微創型,且以樹脂包埋,並施以立即性牙本質封填。利用電腦掃描及電腦製作蠟型,使用熱壓鑄法製作二矽酸鋰陶瓷嵌體,分別以雙聚合樹脂黏著劑(Rely X Ultimate A3, 3M ESPE)、光聚合奈米流動樹脂(Filtek™Z350 XT Flowable A3, 3M ESPE)、光聚合bulkfill流動樹脂(Filtek™ Bulk Fill Flowable Restorative A3, 3M ESPE)黏著,並經過5000次冷熱循環後,在萬能試驗機下進行抗壓強度測試,並在光學顯微鏡和電子顯微鏡下觀察陶瓷破裂的形式。 結果顯示雙聚合型樹脂黏著強度為1002±508 N,光聚合奈米樹脂黏著強度為971±209 N,光聚合bulkfill樹脂黏著強度為581±191 N。根據統計的結果,以光聚合bulkfill樹脂黏著之二矽酸鋰嵌體抗壓強度較低(p<0.05)。破裂型式方面,光聚合bulkfill樹脂黏著之嵌體並沒有牙根斷裂或不可修復的型式發生。

Dental ceramics are not only esthetic, but combined with function and strength. In recent years, minimal invasive dentistry is very popular and going to be the mainstream, including laminate veneers, ceramic inlays, and onlays. As respect to material, there are felspathic porcelain, lithium disilicate and zirconia based prosthesis to choose from. In this study, the concept of minimal invasive treatment was applied, and 24 human maxillary premolars were chosen for research. Teeth were prepared with MOD cavity design, embedded with light-cure resin, and dentin was immediately sealed. These teeth were scanned with extraoral scanner, wax patterns were designed with software, and finally lithium disilicate ceramic inlays were pressed. Ceramic inlays were cemented in 3 groups: dual-cured resin cement, light-cured composite and bulk filled composite. After 5000 thermal cycles, samples were tested in fracture load strength. The interfaces were observed under the stereoscope and field emission scanning electronic microscope, and the fracture types of ceramic inlays were analyzed. The result reveals the compressive strength in dual-cured adhesive resin is 1002±508 N, light-cured nanohybrid flowable resin is 971±209 N, light-cure bulkfill resin is 581±191 N. Based on the statistical result, the fracture load in bulkfill group is significantly lower than other groups. As to the fracture types, the bulkfill group does not have any types of root fracture or unfavorable fracture.
