探討第三型環狀聚合蛋白之單核苷酸變異對於台灣女性罹患子宮頸癌前病變及子宮頸癌的影響

摘要


本研究的目的是探討台灣女性中第三型環狀聚合蛋白(Pentraxin 3;PTX3)基因的單核苷酸多型性(Single Nucleotide Polymorphisms;SNPs)與子宮頸癌的發展和臨床病理特徵以及患者存活率之間的關係。在本研究中,我們分析了125名浸潤性子宮頸癌患者和 98名子宮頸癌前病變患者,以及325名子宮頸抹片檢測正常的女性。我們選擇了四種的PTX3基因單核苷酸變異點,分別是rs2120243、rs3816527、rs2305619和rs1840680,並利用聚合酶鏈反應分析它們的基因型分佈。接著進一步利用卡方檢定分析不同女性族群中的基因表現差異,以及此些基因差異與子宮頸癌患者女性中表現出的臨床病理特徵之間的相關性。接著利用單變項分析及多變項分析檢視不同基因差異及不同臨床病理特徵對子宮頸癌患者五年存活率的影響。我們的結果顯示,PTX3基因單核苷酸變異點rs2120243之基因型為CC的患者相較於基因型為CA/AA的患者罹患腺癌(而非鱗狀細胞癌)的機率更高;而單核苷酸變異點rs1840680之基因型為GG的患者和GA/AA的患者相比亦有更高的機率罹患腺癌。至於其他臨床病理特徵與本研究中所探討的四種PTX3基因之SNPs並無相關,而除了骨盆腔淋巴結的轉移之外,本研究中所探討的臨床病理特徵及四種PTX3基因之SNPs都和子宮頸癌患者的 5 年存活率沒有顯著關聯。總結來說,本研究中所探討的PTX3基因之SNPs與台灣女性的子宮頸癌發生率和患者存活率無關,但和子宮頸癌表現之腫瘤組織型態有關。

This study aimed to investigate the association between single nucleotide polymorphisms (SNPs) of pentraxin 3 (PTX3) gene and the incidence of cervical precancer or cervical cancer in Taiwanese women, as well as the impact of these genetic variations on the clinicopathological features of cervical cancer and patient survival. The study enrolled 125 patients with invasive cancer, 98 with precancerous cervical lesions, and 325 women with normal cervical cytology. Four SNPs of PTX3 gene rs2120243, rs3816527, rs2305619, and rs1840680 were selected. We then analyzed the genotypic distribution of these SNPs with real-time polymerase chain reactions. We used the chi-square test to examine the differences in gene distribution among the groups and the effect of gene differences on the clinicopathological characteristics of cervical cancer patients. We then used univariate and multivariate analysis to explore the influence of the genetic variations of PTX3 gene and the clinicopathological factors of cervical cancer on the five-year survival rate. Our results indicated that patients with genotype CC in PTX3 gene rs2120243 and genotype GG in PTX3 gene rs1840680 had more chance of having adenocarcinoma but not squamous cell carcinoma as compared to those with CA/AA and those with GA/AA, respectively. There were no differences in genetic distributions among women with precancerous lesions, invasive cervical cancer, and normal cervical cytology. Other clinicopathological features had no associations with the four SNPs of PTX3 gene investigated in this study. Except for pelvic lymph node metastasis, none of the clinicopathological features and the four SNPs of PTX3 gene discussed in this study had significant associations with 5-year survival in cervical cancer patients. In conclusion, the SNPs of PTX3 gene have associations with histological types of cervical cancer but not patient survival in Taiwanese women.
