探討關節軟骨碎片與滑液膜對於骨髓間葉幹細胞進行軟骨分化的影響

摘要


目前已發展出許多治療軟骨損傷的方式,如骨髓刺激術、微骨折、自體軟骨細胞移植等方式,但在臨床治療上仍有形成纖維軟骨等限制。組織工程是近年來修復關節軟骨的趨勢,但目前還沒有一個最恰當的誘導方式,因此本實驗希望可以延續軟骨組織工程的概念外加關節腔裡的成分如軟骨碎片與滑液膜進一步研究軟骨碎片與滑液膜對於骨髓間葉幹細胞走向軟骨分化的影響。 本實驗取用豬的關節軟骨碎片、滑液膜與骨髓間葉幹細胞利用第一型膠原蛋白包裹後培養28天。從結果顯示滑液膜能夠促進骨髓間葉幹細胞增加醣胺多糖分泌,且促使骨髓間葉幹細胞沿著軟骨碎片邊緣生長;含有軟骨碎片的組別能使第二型膠原蛋白基因表現量有上升的趨勢;添加骨髓間葉幹細胞與軟骨碎片及滑液膜進行共培養時,骨髓間葉幹細胞能夠促進蛋白聚醣與第二型膠原蛋白上升並在培養14天後可以抑制細胞肥大化。顯示軟骨碎片與滑液膜可以誘導骨髓間葉幹細胞走向軟骨分化。 本實驗結果指出骨髓間葉幹細胞與關節腔內成分進行體外共培養,能夠應用於軟骨組織工程中,期望未來能夠進一步結合骨髓刺激術運用在關節軟骨受損的修復。

Various ways have been developed to manage and treat cartilage injuries. For example, bone marrow stimulating techniques, mosaicplasty and autologous chondrocyte implantation. However, current clinical therapies still have some limitations regarding to the formation of fibrocartilage. On the other hand, tissue engineering of cartilage has been recognized as a new treatment for cartilage damage, but it still has some shortcomings. Therefore, we hypothesized the concept of cartilage tissue engineering and the component of the articular such as cartilage fragments and synovial membranes could promote the chondrogenesis of bone marrow-derived mesenchymal stem cells (BM-MSCs). In this study, cartilage fragments, synovial membranes and BM-MSCs were wrapped by collagen type I solution. Histological and GAGs analysis showed that culturing with synovial membranes could increase glycosaminoglycans (GAGs) and promote BM-MSCs which closed to cartilage fragments. Gene expression of groups contained cartilage fragments that can increase collagen type II. When BM-MSCs co-cultured cartilage fragments and synovial membranes, BM-MSCs could increase aggrecan, collagen type II and inhibit hypertrophy after 2 weeks. In conclusion, we demonstrated the effect of articular cartilage fragments and synovial membranes in promoting chondrogenic differentiation of BM-MSCs. We hope this ideal result could develop a novel condition for articular cartilage repairing and combine bone marrow stimulating techniques which able to apply on clinical treatment in the future.
