臥床時間對於急診血壓偏高病患血壓之影響及其相關因素之探討

摘要


本研究目的為探討急診求診之血壓偏高病患於臥床期間的血壓變化。於96年9月至11月間,在北部某教學醫院之急診室中,共86位血壓偏高病患完成本研究。在病患臥床90分鐘期間,每10分鐘測量一次血壓,以「疼痛等級量表」及「情境焦慮量表」做為測量病患的疼痛及焦慮程度。研究資料以描述性統計及混合模式(mixed model)分析。 結果發現如下:(1)急診血壓偏高病患在不同臥床時間下血壓值呈現顯著變化。(2)在最後混合模式下,時間與年齡影響收縮壓變化;時間、年齡、性別及焦慮程度影響舒張壓的變化。(3)病患於臥床第30分鐘後其收縮壓到達平穩,臥床第20分鐘後舒張壓呈現穩定。 根據本研究結果,建議急診求診中之血壓偏高病患經過臥床後,再次測量血壓時間的標準,進而接受或調整高血壓相關治療。期能藉此研究作為臨床急診護理人員照護之參考。

The purpose of this study was to explore the effects of time course of bed rest on patients with high blood pressure in emergency department. A sample of 86 patients with high blood pressure completed this repeated measurement study in an emergency department in a teaching hospital in northern Taiwan from September 2007 to November 2007. We measured each patient’s blood pressure every 10 minutes for a period of 90 minutes. During the 90 minutes of bed rest, the Numeric Rating Scale and State Anxiety Inventory were used to assess the degree of patients’ pain and anxiety. Data were analyzed using descriptive statistics and Mixed Models. The main findings are as follows: (1) the patients’ blood pressure significantly changed after bed rest. (2) In our final mixed model, time and age affected systolic blood pressure while time, age, gender and anxiety affected diastolic blood pressure. (3)The systolic pressure became stable after thirty minutes’ bed rest. The diastolic pressure also became stable after twenty minutes’ bed rest. According to the data, we suggest that for those high blood pressure patients, it may be a better way to follow the blood pressure after bed rest in emergency department, in order to distinguish whether a treatment for the hypertension is needed. We hope that can provide further knowledge basis for emergency nursing.
