摘要

癌症成為臺灣十大死亡原因之首多年，死亡風險高達12.45%。當無法避免死亡時，如何面對是重要的課題，且癌症末期的許多症狀常使病患在死亡前承受許多痛苦，如何度過格外重要。近幾年興起的安寧療護照護模式，是根據癌症末期病人及其家屬的需求提供個別的照顧計畫，滿足病人和家屬身體、心理以及心靈的需要，以減緩症狀取代積極治療。以提升整體的生活品質為目的，幫助病人盡可能的積極生活，直到死亡。
隨著時代的進步，科技大量應用在醫療產業上，而遠距安寧療護也是重點發展之一。居家照護系統可提供病患及其家屬了解居家照護知識，降低無法立即獲得幫助之憂心，幫助病患及其家屬提升居家自我管理能力，更可使病患及家屬在熟悉舒適的環境下度過餘生。但以往的研究指出系統的複雜度以及較不容易理解使用流程，降低了使用率，如何簡化使用流程將是推動居家自我管理的關鍵。
因此，本研究建立之癌症病患居家自我管理系統，協助病患癌症症狀之處理，透過醫師指示或是系統提供建議之各評估項目，定時提醒使用者評估病患自身狀況或是由家屬協助評估，並在評估後給予適當建議，提高使用者症狀觀察力。醫護端也能同步讀取資料給予相關意見，提高癌症末期病患及其家屬的自我管理能力。系統同時將減少以往文獻提供的評估項目之題目數量，簡化使用流程，以提升採取居家安寧照護的意願。系統也提供行動通訊端的介面，使居家醫護人員可以在居家探訪時的資料閱讀。

In Taiwan, cancer has been the top leading cause of death in Taiwan for many years, and the risk of death is 12.45%. If death is inevitable, how to face it is an important issue. During the terminal cancer symptoms, how to spend the time is also important, because patients have to endure a lot of pain before death. Hospice care is a type and philosophy of care that focuses on the palliative care of a terminally ill or patient's symptoms. Aggressive treatment for alleviate the symptoms can make patients have a quality of life and positive living until death.
As time progress, science and technology are widely used in the medical industry. The telehospice is one of the important areas of the high-technology industry. Tele-homecare system can provide patients and their families with knowledge of the homecare to reduce the worry to about helplessness without immediate help, and enhance family self-management skills. It makes patients to pass the rest of their life in a familiar and comfortable environment. However, in previous studies, the complexity and operation difficulty of the system, reduce the utilization rate. How to simplify the process is the key to for promotion of home tele-hospice.
In this study, we develop a self-management system of home-based hospice care for cancer patients. This system can assist patients for the treatment of cancer symptoms, through physician instructions or suggestion of the assessment project and support a timer to remind the users to assess patient's conditions. The conditions will be synchronized into the computers of Medical providers and then advices will be replied to patients after assessing. This system can support the self-management of terminally ill cancer patients and their families. The system reduce traditional assess items, and simplify the process to enhance home tele-hospice. The system also provides a mobile communications interface, for medical stuffs to download and study patients’ information when stuffs visits patients.
