背景 臨床藥師於加護病房之照護對病人治療的成果有極大幫助,故藥師的重症照顧養成教育非常重要。且臨床藥師須協助線上藥師、藥學系實習生及社區藥師等進行藥物教育,工作業務極為複雜與沉重,若能利用資訊與網路科技提供彈性的教學與學習環境,將可以減輕臨床藥師的教學負擔且能增進學生的學習效益。本研究建立重症用藥線上教育訓練系統,提供藥學系實習生和在職臨床藥師的線上學習平台。 方法 本研究將導入的e-Learning系統,以原有的功能進行規劃,及設計藥物動力學計算工具及藥學相關資訊等擴充功能,建立重症用藥線上教育訓練系統。以台北醫學大學藥學系四年級至萬芳醫院藥劑部實習生為對象,進行系統介入前與介入後學習成效評估。實驗對象分為系統介入前傳統的課室教學方式(對照組)與系統介入後重症用藥線上教育訓練系統教學方式(實驗組),運用KAP模式(Knowledge, Attitudes and Practices of Research Scale)建構藥學系實習生知識、態度及執行能力之問卷,並於學習前(第一次)和學習完成後(第二次)實施問卷,利用Wilcoxon Signed Ranks Test進行檢定兩組第一次和第二次的測驗結果。最後,再針對實驗組實習生進行訪談訪分析,瞭解重症用藥線上教育訓練系統對臨床藥學實習的成效。 結果 本研究以重症用藥線上教育訓練系統上線前最後一批實習生為對照組,共11位,和上線後第一批實習生為實驗組,共12位。實習期間實驗組實習生使用重症用藥線上教育訓練系統登入次數平均為25次,且每位實習生閱讀臨床藥學組課程教材的平均總時間約10小時。KAP問卷利用Wilcoxon Signed Ranks Test進行檢定兩組第一次和第二次的測驗結果,對照組實習生在Practice(執行能力)部分,則具有統計上的顯著差異(p=0.011<0.05);而實驗組實習生,則是對於Knowledge(知識) 與Practice(執行能力)上,具有統計上的顯著差異(p值分別為0.000及 0.031)。 結論 本研究結果顯示,經過臨床重症用藥實習後,對照組實習生在解決專業問題之判斷力與執行能力有所提升;不過經由重症用藥線上教育訓練平台教學之實習生對於臨床藥學上所具備的知識以及解決專業問題的判斷力與執行能力(Practice)也有所提升。得知重症用藥線上教育訓練系統對於學生修習臨床重症用藥實習課程具有較傳統教學方式更佳的效益,未來可以推廣至臨床藥師與社區藥師的繼續教育訓練。
Background The conditions of the patients in intensive care units (ICUs) are most critical and complicated. Clinical pharmacists must carefully evaluate each individual’s medication outcomes through a series of processes such as review, analysis, recommendation, execution, monitoring and assessment of medications. Moreover, clinical pharmacists are also responsible for support of daily drug consultations and internship training programs, dramatically increasing their workloads. In this research we developed an online pharmaceutical education systemfor pharmacy practice students in critical care training. It is expected that the system can reduce the clinical pharmacists’ workload and facilitate the students learning. Method Our work will integrate with e-Learning system, reschedule and redesign the existing functions, computing tools and pharmacy related information, and then builds the Online Pharmaceutical Education System. The thesis will evaluate the impact of between with or without the e-Learning system, and all interns are the internship training program students who study Pharmacy department of Taipei Medical University assigned to the Pharmaceutical Department of Wan Fang hospital. They will be distributed into control and experiment groups. We will develop a questionnaire to evaluate the interns’ knowledge, attitudes and performance using the KAP model. And then we will use Wilcoxon Signed Ranks Test verify the results. Finally, In order to find out the impact of integrating with Online Pharmaceutical Education System on Practice in Clinical Trials, we will interview with the participants in the experiment group. Results The control group has 11 interns that the last group before the Online Pharmaceutical Education System is applied. And the experiment group has 12 interns that the first group uses Online Pharmaceutical Education System. During the internship, the experiment group logged in the system average of 25 times, and every intern studied clinical pharmacy courses on the system about 10 hours. The KAP model verifies the first and second testing results by using Wilcoxon Signed Ranks Test. The results show a significant difference in practice by the control group (p=0.011). On the other hand, there is also a significant difference in knowledge and practice by the experiment group (p=0.000, 0.031). Conclusion This thesis result shows some obvious truths, which improve interns’ decision and execution capability while dealing with problem after learning clinical pharmacy. And using “Online Pharmaceutical Education System” also improve knowledge of clinical pharmacy. And the result shows that Online Pharmaceutical Education System is better than traditional teaching method on learning clinical pharmacy for students. In the future, the system can popularized to other educated training for clinical pharmacist.
