整合升糖指數及運動管理之糖尿病隨身照護系統

Design of a portable diabetes care system integrating glycemic index and exercise management

摘要


台灣約有120萬糖尿病人口,且盛行率逐年上升,世界衛生組織WHO推估至2030年全球糖尿病人口將增加至三億六千萬,屆時將造成極大的社會負擔。 多年來,糖尿病持續高居台灣十大死因的第五順位,主要是由於血糖控制不得當,引起多種併發症,造成病情嚴重惡化。為了維持血糖的穩定度,降低或延緩併發症的產生,糖尿病患必須確實監控血糖狀況並做好日常生活管理。 美國糖尿病協會(American Diabetes Association, ADA)2010年所公佈的糖尿病診療指引,於飲食與運動管理部分提到:血糖的控制除了注意碳水化合物的攝取,以升糖指數與升糖負荷作為飲食參考建議,更有助於血糖的控制;運動部分則建議一星期至少從事150分鐘達中等強度的有氧運動及3次的阻力運動。 本研究以美國糖尿病協會所制定的糖尿病診療指引為依據,結合升糖指數概念與有氧運動、阻力運動的互動式運動遊戲 (以Wii Fit為參考),建置行動式自我健康管理系統。使用者透過行動裝置即可隨時查詢食物升糖指數與計算運動消耗卡路里,輔助糖尿病患於日常生活的飲食與運動管理。另外,使用者可透過系統建立健康日誌,記錄日常服用藥物、飲食、運動及生理參數,除了提供照護者作為參考,病人亦可透過系統查看生理參數的趨勢分析,了解自己病情控制的狀況。希望透過系統化的管理模式,能夠提升糖尿病患的自我管理效能。

There is approximate 120 million diabetes population in Taiwan, and the prevalence of diagnosed diabetes has increased steadily. It is predicted that there will be 360 million diabetes population around the world in 2030 by WHO, then diabetes has been responsible for an enormous social burden. In recent years, diabetes is continued the fifth leading cause of death in Taiwan. The major problem is caused by poor blood glucose control which leads to complications. Morning blood glucose and self-management are both important issues in daily life for people with diabetes. American Diabetes Association publish Standards of medical care in diabetes—2010, which recommends that for individuals with diabetes, use of the glycemic index and glycemic load may provide a modest additional benefit for glycemic control over that observed when total carbohydrate is considered alone. In physical activity, people with diabetes should be advised to perform at least 150 min/week of moderate-intensity aerobic physical activity. In the absence of contraindications, people with type 2 diabetes should be encouraged to perform resistance training three times per week. In this study, we developed a healthcare and self-management mobile system based on American Diabetes Association (ADA) guidelines, which integrate concept deployed for glycemic index, aerobic exercise and resistance exercise by interactive sports games ( use Wii Fit as a reference). Users can check food glycemic index and calculate the consumption of calorie by this system, and thus assist diabetes patients in daily food and exercise management. Moreover, users can establish a healthy log by mobile system, recording the daily medication, diet, exercise and physiological parameters. In addition to providing caregivers as a reference, the patient can also view the trend analysis of physical parameters by this system, and understand their disease control condition. We expect the systematic management can improve diabetes patient’s self-management efficiency.
