護理人員接受高級心臟救命術訓練課程立即成效影響因素之探討

Factors influencing the immediate effect of nurses learning from ACLS training course

摘要


本研究採橫斷式研究,旨在探討醫院施行ACLS訓練課程其影響立即成效之相關因素,以參加北部某醫院ACLS訓練且從事護理工作之護理人員為受試者,在排除非全程參加下,共收案145名。研究以「自信心視覺分數量表0-100mm」、「自擬結構問卷」、「高級心臟救命術課程知識前測」、「高級心臟救命術課程知識後測」、「Mega-VF主觀評量表」、「心肺復甦術檢核表」為工具進行測量。 結果顯示:1.收案醫院平均通過率為71.7%。2.參加動機:現職單位評鑑要求、累計繼續教育積分、個人興趣想學習更多急救技能、訓練課程內容符合等。3.課後比課前筆術得分高,課後筆術得分會因職稱、是否參加過訓練而產生差異,並與年齡、年資、課前筆術得分、課前通過及執行自信心、BLS得分、Mega-VF得分、ACLS態度得分、課後通過及執行自信心呈現相關。4.Mega-VF得分與課前筆術得分、課後筆術得分、BLS得分、課後通過及執行自信心呈現相關。5. ACLS態度與課前筆術得分、課前執行自信心、課後筆術得分、課後通過及執行自信心呈現相關。6. 課後通過及執行自信心明顯比課前高。7.課後通過自信心與年資、課前筆術得分、課前通過及執行自信心、課後筆術得分、Mega-VF得分、ACLS態度得分及課後執行自信心等呈現相關。8.課後執行自信心與年資、課前通過及執行自信心、課後筆術得分、Mega-VF得分、ACLS態度得分、課後通過及課後執行自信心等呈現相關。9. 課後筆術得分重要預測因子為Mega-VF得分、有參加過訓練、BLS得分及課後執行自信心。10. Mega-VF得分重要預測因子為BLS得分及課後通過自信心。11.ACLS態度得分重要預測因子為課後通過自信心、課前筆術成績。12.課後通過自信心重要預測因子為課後執行自信心、課前通過自信心、ACLS態度得分及課前執行自信心。13.課後執行自信心重要預測因子為課後通過信心程度、課前執行自信心、參加臨床急救經驗三年以上、N2、課前通過自信心、區域醫院等。 本研究為描述ACLS訓練影響因素初探,綜觀國內並無此議題之研究,雖然本研究有許多的限制,並礙於人力、時間及資源等,但其研究結果仍可做為未來研究或舉辦此類課程之參考。

The purpose of this study was to investigated the factors influencing the immediate effect of nurses learning from Advanced Cardiac Life Support (ACLS) training course. This is a cross-sectional study, convenient sampling was conducted, nurses participate in a regional medical ACLS training in the north bond but exclude non-full participation, and 145 questionnaires were analyzed. The research instruments administered were as follows: (1) Confidence visual point scale 0-100mm (CVPS); (2) Structural questionnaire; (3) Pre-course multiple choice quiz (pre-MCQ); (4) Post-course multiple choice quiz (post-MCQ); (5) Mega-VF subjective rating scale (MVRS); (6) CPR- checklist rating scale (CCRS) . The main results are as follows: (1) The average pass rate of regional hospital ACLS training course is 71.7%; (2) The main reasons for participating the ACLS training include: evaluation requirements of serving units, continuing education points, to learn more first aid skills, the training course meet their need, and gaining salary if received ACLS certificate; (3) Post-MCQ score was significant higher than pre-MCQ score. Post-MCQ score is due to title, whether prior training experience participated in ACLS to differ, and age, work experience, pre-MCQ score, pre-training self-pass-confidence visual point scale (pre-P-CVPS), self-confident of pre-training implementation (pre-I-CVPS), CCRS, MVRS, ACLS attitude scores (AS), post-training self-pass-confidence visual point scale (post-P-CVPS), and self-confident of post-training implementation (post-I-CVPS) are correlated; (4) The correlation of MVRS, pre-MCQ score and post-MCQ score and CCRS and post-P-CVPS and post-I-CVPS are significant; (5) AS and pre-MCQ score, pre-I-CVPS, post-MCQ score and post-P-CVPS, and post-I-CVPS are highly correlation; (6) The post-P-CVPS and post-I-CVPS is significantly higher than the pre-P-CVPS and pre-I-CVPS; (7) The correlation of post-P-CVPS, working experience, pre-MCQ score, pre-P-CVPS, pre-I-CVPS, post-MCQ score, MVRS score, AS and post-I-CVPS are significant; (8) The correlation of post-I-CVPS, work experience, pre-P-CVPS, pre-I-CVPS, post-MCQ score, MVRS score, AS, post-P-CVPS and post-I-CVPS are significant; (9) The significant predictor of post-MCQ score was MVRS, trainees with ACLS experience, CCRS and post-I-CVPS; (10) The significant predictor of MVRS was CCRS and post-P-CVPS; (11) The significant predictor of AS was post-P-CVPS and pre-MCQ score; (12) The significant predictor of post-P-CVPS was post-I-CVPS, pre-P-CVPS, AS and pre-I-CVPS; (13) The significant predictor of post-I-CVPS was post-P-CVPS, pre-I-CVPS, staff with ACLS more than 3-year experience, N2 level competence and local hospital. This study describes the factors that affect the ACLS training; there is no further research about this issue in Taiwan. Although manpower, time and resources are the limitation, but the findings could be a recommendation for manage an ACLS course or further research.
