In-flight and ground safety is the manifestation of both integrative training result and general performance evaluation for the aviation industry. In spite of the enforcement of numerous flight-safety-prevention related measurements, we still have witnessed several flight safety indicants/violations of various degrees of severity. As a result, what and how to do to promote flight safety remains a key issue for the aviation industry. Human factor is more or less connected to virtually every aspect, given the dauntingly complex flight mission environment we face today. According to data related to several major flight safety incidents of ROCAF in the time span from 2001 and 2004, it suggests that much still need to be addressed and improved as far as flight safety checking system is concerned. Furthermore, based on statistics data relating to flight safety for the last 10 years, we’ve seen a steady trend of increase for the “Category 4” safety incidents/violations. How do we explain this? Perhaps human error is one of the factors to account for such unusual phenomenon. Following the increase of both flight sorties and mission frequency, we can expect more flight incidents in the future if no significant improvement related to flight safety standard can be solidified. This study, consequently, addresses the dimensions of “base culture” human fatigue” personal awareness” and “interpersonal communication”. We hope that we can come up with some useful advice or recommendation to contribute to the flight safety prevention initiatives of the ROCAF
