應用於糖尿病患者之遠距居家照護系統的開發與評估

Development and evaluation of a home teleheath system for diabetic patients

摘要


高齡化社會中,遠距居家照護系統(home telehealth system)的應用,已經廣泛受到重視。本研究以「可攜式遠距居家照護系統(Portable Tele-homecare Monitoring System, PTMS)」為基礎架構,開發了一套的遠距居家照護系統,應用於糖尿病患者之照護。PTMS與一般遠距居家照護系統最大的不同,是其以每一家庭(household)為健康監測資料傳送、儲存、與分析的基本單位,高度分散式資料儲存與可攜式之特質,取代了一般系統集中式資料庫管理方式,可依個人需求以低廉的成本裝置於居家環境中,是在經濟上可實行且為用戶端所能接受的系統。 本系統實際應用於敏盛醫院的6位糖尿病患者上,並針對系統進行使用者接受度、臨床結果與經濟上的初步評估。系統提供一個工具可讓病患紀錄他們的生理資訊,同時也讓醫生與照顧人員來管理與教育他們的病患,實際應用情境下的測試中也發現本系統仍有一些技術限制與需要改善之處。本論文最後並討論在PTMS架構下,遠距居家健康系統可能的“Direct-to-Consumer Approach”之營運模式。

The advantage of using the telehealth systems for diabetic patients care has been widely recognized. In this study, a home telehealth system based on the Portable Tele-homecare Monitoring System (PTMS) structure for diabetic patient care has been developed. What sets this work apart from most other systems is the focus on a highly decentralized monitoring model and the portable nature of the system. Instead of using a centralized database structure that gathers data from many households, in the PTMS a household is the fundamental unit for data transmission, storage, and analysis. The PTMS can be easily adapted and implemented in a home environment with very low costs. We believe that this is the approach that is needed to make such systems economically viable and acceptable to the end-users. This system was applied to 6 diabetic patients of Ming-Sheng Hospital for a preliminary assessment of its user acceptance, economic evaluation and clinical outcome. The system provides a tool for patients to recording their vital sign measurement data, and for doctors/care-givers to monitor and educate their patients. However, there are also several limitations to the system. Finally the “Direct-to-Consumer Approach” business model for home telehealth systems using this structure is described.
