探討護理人員之病人安全認知及醫院安全文化之看法

摘要


本研究目的在探討護理人員病人安全認知、病人安全文化及其影響因素。採橫斷式相關性研究法,以方便取樣方式選取台灣南部某地區及區域醫院護理部的護理人員及護理主管為研究對象,問卷發放363份問卷,回收有效問卷328份,有效回收率為90.36%。研究工具為結構性問卷,包括基本資料、病人安全認知量表、和醫院病人安全文化調查問卷。所得資料以獨立樣本t檢定、單因子變異數分析、皮爾森積差相關即逐步複迴歸等方法進行分析。結果顯示護理人員之個人基本屬性中的職別、宗教信仰、子女數、進階層級、是否固定人力、是否固定班別以及服務醫院,與病人安全文化的評量有關;醫院部門間的合作關係及部門間資訊的交換過程,與護理人員的病人安全認知有關;影響病人安全文化之重要預測因子為:進階層級、固定班別、固定人力及職別等;整體護理人員認為醫院病患安全等級在可接受以上範圍。根據本研究結果,醫院管理階層應重視護理人力問題以維護病人安全及護理品質,促進部門間合作以營造機構整體病人安全文化,並建立病人安全管理系統,落實病人安全執行。建議未來研究可設計醫院病人安全文化之介入性方案並評值其成效,以提升照護品質。

The aim of the present study was to examine the patient safety perception and patient safety cultures from Nurse’s perspectives. In this cross-sectional correlation study, data were collected by structured questionnaires. The questionnaire was distributed to 363 nurses in two hospitals in southern Taiwan; 328 nurses completed and returned the questionnaire for a valid response rate of 90.36%. The dependent variables were measured by patient safety grading system and hospital patient safety cultures questionnaire. Independent sample t test, One-way ANOVA, Pearson’s correlation coefficient, and stepwise multiple regression were used to analyze data,. The results showed that factors affecting nurses’ assessment of patient safety culture included occupation, religion, number of children, clinical ladder, part-time or full-time job, regular shift or not, and hospital standard. In addition, the related factors of patient safety perception included cooperation between department and information exchange. Stepwise multiple regression analysis showed that the best subsets to predict positive patient safety culture included higher clinical ladder, fixed regular shift, fixed department, and nursing managers. The overall assessment of patient safety grade was acceptable. Based on the findings, hospital management must value nursing manpower to enhance patient safety and nurse care quality, promote cooperation between departments to build patient safety culture, and establish patient safety management system in order to fulfill the implementation of hospital patient safety environment. It is suggested that further research should design and evaluate the effects of intervention protocols of patient safety culture.
