長照2.0社區整體照顧服務體系A級個案管理員專業能力觀點之探究

摘要


臺灣於2016年底開始推動長照2.0政策,當中社區整體照顧服務體系A級社區整合型服務中心編置有個案管理員,個管員依照管專員核定之額度及問題清單,為失能者擬定照顧服務計畫及連結或提供長照服務,進行照顧管理工作,在長照2.0中扮演關鍵及重要之角色。 本研究運用半結構式訪談,並以立意抽樣之方式針對8位含括高雄市7大分區、不同單位屬性及不同專業別之A級單位主管及督導進行訪談。研究發現A級個管員專業能力可分為知識、技術及態度三大部分:知識能力包含服務對象、長照服務及資源管理;技術能力包括評估、溝通、整合及倫理能力;態度能力則有正向與彈性、積極與投入、誠實與穩定,以及獨立與創新。故依此研究結果提出研究建議:一、建立公私部門參與溝通對話機制;二、訂定A級個案管理員專業能力指標;三、依據專業能力指標設計A級個案管理員訓練;四、擬定A級個案管理員倫理守則。

In the end of 2016, Taiwan government kicks off Long Term Care Plan 2.0. Under the new policy, the Tier A—community integrated service center within the overall long-term care community service system sets up case managers. Within the pre-approved guidelines of financial allowance and individual demands from supervisors, case managers provide care plans and services for people with disability, and therefore play the key role in Long Term Care Plan 2.0. This research conducts semi-structured interviews. With the purposeful sampling, I hold interviews with 8 senior directors and supervisors with various institutions and expertise from 7 districts of Kaohsiung. My findings can be distributed into three major categories of knowledge, technique and attitude. Knowledge indicates the understanding of people with needs, long term care service and resource managements. Technique indicates the ability of evaluation, communication, integration and ethics. Attitude indicates positive thinking, flexibility, energetic behavior, devotion, honesty, stability, independence and innovation. Based on research results, I provide suggestions: 1.To establish communication systems between the government and private institutions. 2.To define personal assessment criteria of Tier A case managers. 3.To design personal training programs based on assessment criteria for Tier A case managers. 4.To outline ethic principles for Tier A case managers.
