本研究目的在探討專科護理師與其共事主治醫師對專科護理師角色功能及角色能力表現認知之差異及相關影響因素。採橫斷性研究設計結構式問卷進行調查。研究對象為南部某醫學中心及二所區域教學醫院中從事職務3個月以上之專科護理師及當月共事之主治醫師。回收205份有效問卷含專科護理師112份(96.55%)及主治醫師93份(86.58%),研究結果發現:(1)專科護理師認同醫療輔助者、溝通協調者、直接照護者、人文關懷者、實務指導者、倫理決策者、專業諮商者、臨床研究者及領導改革者等九大角色功能;主治醫師認同專科護理師應具備醫療輔助者、溝通協調者、直接照護者、人文關懷者及實務指導者等五大角色功能。(2)專科護理師與主治醫師均認同專科護理師已具備溝通協調、倫理決策、實務指導、專業諮商、人文關懷等能力表現,而領導改革與臨床研究能力表現是較不足的。(3)專科護理師角色功能之影響因素有任職科別及目前在該科年資;能力表現之影響因素有任職科別、擔任專科護理師總年資、目前該科年資、教育程度及在職教育頻率。研究結果可提供專科護理師角色能力發展的參考。
This study is to explore the variation of cognition on role function and role competence of nurse practitioner and the influence factors by the self of nurse practitioner and collaborating physician. A cross-sectional research design with structured questionnaires was used for collecting data. The subjects selecting from one medical center and two regional teaching hospitals in south of Taiwan included nurse practitioners who are employed more than three months and their collaborating physicians in the period of data collecting. Total 205 valid questionnaires were from 112 nurse practitioners (96.55%) and 93 collaborated physician (86.58%). The results showed (1) Nurse practitioners recognized nine role functions including medical assistant, coordinator, direct care provider, humanitarian, practical guide, ethical decision-maker, professional consultant, clinical researcher, and reform leader. Their collaborating physicians recognized five role functions of nurse practitioners including medical assistant, coordinator, direct care provider, humanitarian, and practical guide. (2) Both of nurse practitioners and physicians recognized the competence of nurse practitioner in communication and coordination, ethical decision- making, practical guidance, professional consultation, and humanity. However, the abilities of clinical research, and reform leadership were insufficient. (3) The influence factors of role function of nurse practitioners were department of service and the employment years as nurse practitioners. The influence factors of role competences of nurse practitioners were department of service, the employment years as nurse practitioners, the education and frequency of in-service education. The results of this study could used to support the role development of nurse practitioners.
