肥胖與正常體重者於運動後恢復期使用慢速呼吸對自主神經活性之影響

背景與目的:健康人運動後需要一段時間自主神經活性方能恢復至運動前狀態,過去研究顯示肥胖者自主神經活性有失調現象,但對於此族群運動後自主神經恢復反應則仍未有研究探討。慢速呼吸可用來提升副交感神經活性,是否能夠幫助運動恢復期副交感神經之回復仍不可知。因此,本研究欲探討肥胖者與正常體重者運動後恢復期自主神經活性調控情形,以及慢速呼吸對運動後自主神經恢復之影響。方法:招募40位無明顯身體疾病的健康成人,肥胖組與正常體重組各20位,進行三分鐘登階運動。每位個案接受兩次測試,隨機分配決定於運動恢復期使用慢速呼吸與否。運動前五分鐘、恢復期第15-20、30-35、45-50及60-65分鐘收集於控制呼吸下之心率資料進行事後分析,以了解恢復期各階段自主神經活性(總功率、高頻、低頻、標準化高頻、標準化低頻及低高頻比率)。結果:高頻參數於運動後第15分、30分、45分及60分肥胖組均顯著低於正常體重組(p<0.01),低頻參數於各測量時間點無顯著差異。肥胖組之自主神經活性於運動後45分鐘方回復至運動前基礎值,而正常體重者於第30分鐘即已恢復。不論肥胖組或正常體重組,在執行慢速呼吸及自主呼吸下之自主神經恢復情形類似(p>0.05)。結論:肥胖者於運動後的自主神經活性恢復較正常體重者緩慢,慢速呼吸無法加速運動後自主神經活性的恢復。

Background: After exercise, it takes time for autonomic nervous system(ANS)to recover to the level of baseline in healthy individuals. Previous studies have shown that ANS dysfunction is found in obesity, but the ANS recovery after the exercise in obesity remains to be further studied. Slow breathing can be used to enhance parasympathetic activity. Whether slow breathing can facilitate the recovery of parasympathetic nervous system after exercise is still unknown. Therefore, this study was to investigate the recovery of ANS activities in obese and normal-weight subjects after exercise and the effects of slow breathing on the ANS recovery. Method: Forty healthy adults with no obvious physical illness were recruited, including 20 obese and 20 normal-weight subjects. Each subject performed two trials of a three-minute step test. The test order was randomized based on whether slow breathing was used or not during exercise recovery. Heart rates under controlled breathing were collected at 5 minutes before exercise, 15-20, 30-35, 45-50, and 60-65 minutes during recovery and data were analyzed afterwards (total power, low frequency, high frequency, normalized high frequency, normalized low frequency and low/high frequency ratio). Result: High frequency parameters at the 15thmin, 30thmin, 45thmin and 60thmin in obese group were significantly lower than those in the normal-weight group (p<0.01). There were no significant differences on low frequency parameters at each recovery time point. The ANS returned to the baseline at the 45thmin for the obese group, while the 30th min for the normal-weight group. For each group, ANS recovery under the slow breathing and spontaneous breathing conditions were similar (p>0.05). Conclusion: The ANS recovery after exercise is slower in obesity than in normal-weight individuals. Slow breathing cannot accelerate the recovery of ANS activity after exercise.
