維生素D濃度與生活品質探討

摘要


前言 維生素D為脂溶性維生素,亦是人體重要維生素之一。近來研究發現缺乏維生素D除了在骨骼方面的影響外,也有可能增加罹患其他疾病的風險。維生素D一旦缺乏,易導致副甲狀腺分泌的荷爾蒙增加,過於活躍的副甲狀腺常與憂鬱症有關,曾有研究顯示憂鬱症和體內維生素濃度過低有顯著關係。本研究擬初步調查在南臺灣醫院工作人員的維生素D濃度分布現況,評估其是否受性別、年齡、教育程度、工作性質、疾病等風險因子影響,並探討和生活品質是否有相關。 方法 本研究對象為台灣南部一區域教學醫院參加102年度健康檢查之員工,且有採檢25-hydroxyvitamin D濃度者,以台灣簡明版世界衛生組織生活品質問卷(WHOQOL-BREF)進行調查,共回收161份有效問卷執行分析。後續利用t檢定、單因子變異數分析、皮爾遜積差相關等統計方法進行資料分析。 結果 研究結果顯示,受試者平均維生素D濃度皆為不足情形,但男性濃度略高於女性。不同教育程度及職業類別的維生素D濃度有顯著差異。在生活品質分析方面,維生素D濃度不足對整體生活品質得分無影響。但維生素D濃度在滿意健康、環境範疇及健康相關生活品質滿意程度有統計顯著相關,濃度愈低得分就愈低。 結論 在醫院環境工作者發現維生素D濃度普遍為不足情形,雖然對生活品質無影響,但應關注此健康問題。

Objective Vitamin D is a fat-soluble vitamin, and it is one of the important vitamins in human body. Previous studies have found that deficiency of vitamin D not only influence bones, but also increase the risk of other diseases. Once there is a lack of vitamin D in human body, it might lead to an increase of hormone that released by the parathyroid. In addition, overactive parathyroid correlates highly with depression, and previous studies also indicated that depression correlates highly with low vitamin D concentration in human body. This study is a preliminary research on the distribution of vitamin D concentration in hospital employee of southern Taiwan, and we’d like to evaluate whether it is influenced by risk factors such as gender, age, education level, type of work, and diseases, and investigate whether it is related to quality of life. Methods The study population included those employees who underwent 102 annual physical examinations in study hospital, and among those who had the subject 25-hydroxyvitamin D concentration data. The World Health Organization Quality of Life Questionnaire (WHOQOL-BREF, Taiwan version) was utilized to collect data, and 161 questionnaires were available. The t-test, One-Way ANOVA and Pearson correlation analysis were used to analyze data. Results The results showed that all the subjects vitamin D concentration were insufficiency, but male slightly higher than female. Vitamin D concentrations in different level of education and occupational groups have significant differences. In the analysis of quality of life, vitamin D concentration insufficiency not effect on the overall score of quality of life. But the concentrations of vitamin D in satisfactory health, the environment domain and health-related quality of life satisfaction were statistically significant correlation and the lower concentration with lower score. Conclusion In this study hospital workers were found that vitamin D concentration were insufficiency, although not effect on quality of life, but should be concerned about this health problem.
