砷在全球是一個普遍存在的金屬,在台灣最初的研究則是有報導了肝癌與飲用水裡的含砷量之關係,而因為目前都是在討論砷與肝功能或者是肝功能與CYP2E1之間的關係,很少討論三者之關係。因此本研究目標在探討CYP2E1與砷毒性以及白蛋白之間的關係。本研究使用台灣人體生物資料庫(Taiwan BioBank, TWB) 30至70歲成年人之資料與血漿,血漿以電感耦合電漿體質譜法(Inductively coupled plasma mass spectrometry, ICP-MS)測量取得,資料以健康檢查方式獲得血液及尿液生化值,問卷收集測量人口學資料、健康資料。除描述性統計分析以外,使用線性回歸去分析。經排除B型肝炎帶原及可能感染C型肝炎者及資料不完整,TWB SNP chip之SNP型QA/QC排除16人。最後得到423人,血漿砷平均值±標準差為4.01±1.88 ug/L,調整性別、年齡、BMI、抽菸、飲酒習慣後之複回歸分析,發現血漿砷對健康檢查結果與肝功能指標中僅對白蛋白之迴歸係數有意義(ß=0.0120,p=0.001)。此外白蛋白與基因型CYP2E1的不同型有統計上顯著意義,在白蛋白的部分是rs3747881-CT/TT。本研究發現血漿砷每增加 1 ug/L,白蛋白增加0.022 g/dL(p=<0.05),有rs3747881-CC型的人迴歸係數為0.277 (p<0.05),故rs3747881的CC型是一個易感性基因。
Arsenic is a ubiquitous metal in the world. The original study in Taiwan reported the relationship between liver cancer and arsenic in drinking water, because it is currently discussing arsenic and liver function or liver function and CYP2E1. The relationship between the three is rarely discussed. Therefore, the aim of this study was to investigate the relationship between CYP2E1 and arsenic toxicity and liver function. This study used Taiwan's Human Biobank (TWB) data from 30 to 70-year-old adults and plasma. Plasma was measured by Inductively coupled plasma mass spectrometry (ICP-MS). The health check method obtained blood and urine biochemical values, and the questionnaire collected and measured demographic data and health data. In addition to descriptive statistical analysis, linear regression was used to analyze. The exclusion of hepatitis B with and without hepatitis C was incomplete, and the SNP type QA/QC of TWB SNP chip excluded 16 people. Finally, 423 people were obtained. The mean value of plasma arsenic ± standard deviation was 4.01±1.88 ug/L. After adjusting the gender, age, BMI, smoking and drinking habits, the regression analysis showed that plasma arsenic was in the health check results and liver function indicators. Only the regression coefficient for albumin was significant (ß=0.026, p=0.001). In addition, the different types of albumin and genotype CYP2E1 are statistically significant, and the part of albumin is rs3747881-CT/TT. This study found that for every 1 ug/L increase in plasma arsenic, albumin increased by 0.022 g/dL (p=<0.05), and the regression coefficient of rs3747881-CC type was 0.277 (p<0.05), so the CC type of rs3747881 was a Susceptibility genes.
