多元複合預防介入模組之成效初探-以社區衰弱老人與輕度認知障礙老人為對象

摘要


臺灣正面臨人口老化問題,西元2026年達到超高齡社會。本研究主要目的為探討以職能為基礎的延緩失能社區介入不同模組,本土化後之多面向健康促進成效。本研究為前瞻性類實驗設計,進行時間為民國107年4月13日至108年2月28日,以55歲以上共21名的中老年人為研究對象,其中包含輕度認知障礙、衰弱老人,使用方便取樣的方式進行抽樣,在「衰弱老人生活型態再造」與「健康職能生活處方」等介入模組介入前後,進行前測、後測,與六個月的追蹤,評估內容包含生活品質、有意義的活動參與、身體功能、認知功能等面向。以平均值、標準差、配對樣本t檢定、獨立樣本t檢定、與單因子變異數分析進行資料分析。研究結果發現,從立即成效觀點來看,「衰弱老人生活型態再造」介入模組在生活品質、有意義的活動參與、身體功能、認知功能等面向有顯著成效,「健康職能生活處方」介入模組介入後有維持能力;從維持成效來看,兩個模組在六個月的追蹤後有維持成效。整體而言,兩種介入模組介入後,社區老人可以減緩老化的速度。

In 2026, Taiwan will be a super-age society and face to problems of aging population. The purpose of this research is exploring effectiveness two difference modules with occupation-based intervention, and the effect of localization. This research is a prospective study and quasi-experimental design, started in 2018/04/13 and ended in 2019/02/28. Research object are 21 community elderly who are 55 years old and over. They are mild cognitive impairment or frailty. Researchers use convenience sampling to sample. In this research, include pretest, protest and follow after test in six month. Measurement have four types: quality of life, meaningful activity participation, body function and cognitive function. Statistical analysis use mean, standard deviation, paired sample t-test and independent sample t-test. In immediate effect, module of “Lifestyle modification program for age-related frailty” have significant effect in all aspect, module of “The multi-domain, health promotion program” maintained effect after intervention. In continued effect, two modules all have significant effect. Summary, after intervention of two modules, community elderly can decline aging rate.
