裝戴植牙式牙冠牙橋患者的口腔健康相關活品質及滿意度探討

摘要


研究背景: 由於近年來,在網路資訊的發達及醫療水準的不斷提升,使得台灣國人越來越重視牙齒及口腔保健的問題,雖然口腔的疾病不直接影響危害人命,而口腔健康的相關功能可能會間接影響咀嚼功能、社交生活、發音及心理上而影響生活品質,植牙治療除了可以瞭解及解決患者的口腔問題外,也需要瞭解經過植牙治療能否提升患者的口腔健康相關生活品質。 研究目的: 本研究目的針對牙科患者裝戴植牙式牙冠牙橋前與裝戴後口腔健康相關生活品質及滿意度進行探討。 方法: 本研究屬於追蹤型研究,收案對象為高雄市某牙科診所針對 20歲以上植牙患者裝戴牙冠牙橋裝置以問卷訪談方式進行評估,問卷調查由過經教育訓練之牙醫助理員詳細說明問卷內容及填寫方式,於現場解釋及說明問卷內容(OHIP-14、植牙滿意度)及各個項目協助患者充份了解填寫,分別利用描述性統計、t-test、paired-t test、ANOVA及linear regression及effect size進行分析來探討植牙滿意度與口腔健康相關生活品質。 結果: 在經過臨床6個月植牙治療後,68位牙科植牙患者的口腔健康相關生活品質在功能限制、身體疼痛、心理痛苦、身體限制、心理限制、社交限制、殘障面項中皆有顯著的差異(p<0.001),在植牙滿意度與OHIP-14中,在咀嚼能力、假牙穩定度及醫師服務中有顯著的差異性,在植牙裝完牙冠牙橋裝置後比原本沒有植牙時口腔健康相關生活品質有明顯的改善。 結論: 本研究發現在OHIP-14的 effect size有高的改變量,顯示在牙科植牙治療前與牙科植牙治療後有極高的改變量,也適合植牙需要長時間追蹤與測量口腔健康相關生活品質,且能了解牙科植牙患者在植牙治療後口腔健康相關生活品質的表現是否能改善,而在滿意度的表現上發現牙科患者對於植牙都給予極高的評價,顯示植牙這部分在社會已有良好的評價。

Background:Recently, with the growth of internet information and the improvement of medical care, Taiwanese emphasize problems about teeth and dental care. Even though dental problems will not directly endanger human life, they will indirectly influence chewing functions, social life, pronunciation, and life quality. Implant cure can solve dental problems of the patients and their improve life quality. Objectives:The aim of this study is to investigate the patient’s satisfaction of implant-supported crowns bridge and oral health related quality of life before and after. Methdology : In this follow-up study, the participants who were implant patients with dental crowns and bridge older than 20 years old from a dental clinic in Kaohsiung city were required to complete a questionnaire and participate in an oral interview. Questionnaire survey was conducted by dentist assistants who explained the contents including OHIP-14 and implant satisfaction and the way of rating the scale in detail. The results of the survey were analyzed by descriptive statistics, t-test, paired t-test, ANOVA linear regression, and effect size. Results : After six months, the improvements of 68 implant patients’ life quality were significantly different in function limitation, physical pain, psychological discomfor, physical disability, psychological disability, social disability and handicap (p<0.001). The significant differences were also found in implant satisfaction and OHIP-14, chewing function, denture stability and doctor service. That is, the obvious improvement was found after implant. Conclusion : The results of this study found apparent change in OHIP-14 and effect size to present patients with implant care were better than before. Implant needs long term follow-up and assessment for oral health related quality of life. The patients indicated their high satisfactions in implant. Accordingly, implant is highly accepted in this society.
