配戴全口活動義齒患者使用假牙黏著劑之滿意度調查

摘要


背景: 在面對一些齒槽嵴嚴重萎縮的患者時,如何使全口活動義齒的穩定性及固持力達到滿意是非常困難的課題,此時如果能有其他輔助治療方法的話,將會使病患獲得功能更良好的義齒,因此,發展將近一世紀而不斷改良的假牙黏著劑應可扮演著一個重要的角色。 研究目的: 本研究便是要調查假牙黏著劑的使用情形及它是否能在牙醫師客觀評估和受試者主觀評分中,得到改善義齒功能的正面結果,以期能找出推廣假牙黏著劑的正確方向。 研究方法: 調查來自配合醫療機構中求診的63位全口活動義齒配載患者,徵求其同意後進行受試者基本資料、義齒和假牙黏著劑使用狀況及義齒滿意度的問卷填寫,記錄義齒承載組織評分、使用假牙黏著劑前後義齒評分的差異;在一個月後再進行電訪詢問填寫假牙黏著劑使用情形及義齒滿意度的問卷,將所得資料經由統計分析後以獲得關於客觀的義齒承載組織評分、義齒評分及主觀的受試者滿意度評分在使用假牙黏著劑前後會否有明顯改變。 結果: 本研究的六成受試者有義齒上的不適,大部份原因為義齒易脫落及疼痛。而一開始假牙黏著劑使用率不到四成,經由牙醫師專業建議更只有兩成,使用假牙黏著劑的受試者表示改善最多的地方為義齒不易脫落及易於進食。在客觀的評分後,發現使用假牙黏著劑後義齒分數有顯著提升(p <0.05)且提升量與承載組織及義齒分類相關(p<0.05)。一個月後持續使用假牙黏著劑的受試者約為六成,而在進食、說話、笑容(展示義齒)、清潔、食物塞入義齒下方及整體滿意度等主觀分數方面,一個月後持續使用者在統計上皆有顯著地增加,那些不願使用假牙黏著劑的受試者表示義齒功能良好為主要因素。統計後發現客觀的承載組織分類與義齒的分類與一個月後持續使用假牙黏著劑之間沒有相關性。 結論: 使用假牙黏著劑後不論在客觀評估及主觀評分,皆能有效改善義齒的使用狀況,進而改善病患的飲食狀況及生活品質,然而客觀評估承載組織及義齒後,發現不良的狀況並不會影響患者使用假牙黏著劑的意願。如何正確教導推廣假牙黏著劑給願意並需要使用的患者將是一個重要的課題。 關鍵詞: 全口活動義齒患者、假牙黏著劑、滿意度

Background: How to get more stability and retention in denture when facing the patient with severe ridge absorption had been a hard work.If combined with other auxiliary treatment, it would make patient more satisfied with their denture. Therefore, the denture adhesive which had been ameliorated unceasingly nearly a century would be an ideal choice. Objective: The research was planned to survey the usage of denture adhesive in Kaohsiung city and Ping-Tung county .The objective evaluation of denture and the alveolar ridge made by the dentist and subjective score of denture marked by the participants were also included. Subjects and methods: The research included 63 participants who wearing complete denture from the cooperative hospitals and clinics. The basic document of the participants and their dentures, the usage of adhesive, the objective evaluation of denture and alveolar ridge, the subjective score of denture before/after using adhesive were recorded. After one month, the usage of adhesive and the subjective score of denture and adhesive was traced by telephone visit. All data was analyzed by statistical software to estimate the degree of satisfaction with denture adhesive. Result: Almost sixty percentages of participants had problem about dislodgment and pain when wearing denture. Forty percentages of participants who were used to applying adhesive and half of them were suggested by dentist. The improvement in eating and dislodgement were affirmed by most users. After one month, sixty percentage of participants used adhesive continually. The increase in subjective score about eating, speaking, and smile, cleaning of denture, food impaction under denture and overall degree of satisfaction was significant. And the chief reason of the other participants why abandoning adhesive was their denture could satisfy them already. There was no relation between classification of denture or alveolar ridge and the will to using adhesive. Conclusion: The denture adhesive afforded significant aid to complete denture wearers subjectively and objectively. It would make patients enjoy their life more confidently. Therefore, denture adhesive deserved to be popularized correctly to the denture wearers who need and be willing to use it. Keywords: Denture adhesive, Complete denture wearers、Satisfaction
