牙周補綴之病例報告

中文摘要牙周補綴：最主要是針對嚴重的牙周病，所從事的補綴治療。在嚴重牙周病的治療中，復形和補綴治療是絕對必需的，而這些治療的觀念、原則和技術，一樣可以應用於一般正常的齒列，這是Ｄr . Amsterdam對「牙周補綴」所下更精闢的定義。當然嚴重牙周病與一般正常的齒列的治療考量是有所差異的，因為嚴重牙周病除了傳統的牙周治療外，有的還須結合根管治療、齒顎矯正、口腔外科、人工植牙，甚至是牙科技工，由最傳統單打獨鬥治療到結合各專科合作的「整合性治療」，使其達到最完美的境界。補綴物的完成是治療的最終目的，在這之前利用各種方法來治療控制牙周病，因隨著牙科治療技術的日新月異，牙周病的治療不單只是非手術治療或傳統的牙周手術治療（包括切除性手術或重建手術），更有所謂引導組織再生術、引導骨生成術、牙周整形術的發展，這些治療技術更能提供我們達到牙周治療的目的。改善牙周的情況，擁有健康的牙周環境後，方能做出適當的牙周補綴，讓患者恢復其功能與美觀。　本文提供9個病例報告，三位女性、六位男性，年齡從47至71歲，患者均為中度至重度牙周炎，都經過各階段的完整治療再經過評估訂定牙周補綴計劃，方式從固定式義齒贗復、磁性附連體活動義齒、雙重冠覆蓋義齒、覆蓋義齒，及人工植牙。目前這些案例都能符合患者咀嚼、美觀、發音上的需求，但這些不同補綴設計，是否能長時間讓牙周仍然健康，就需要長期的追踪觀察了。牙周補綴的治療是全方位的考量，“用心”去幫患者思考符合他的治療計劃，解決他的問題，讓他對我們有信心，進而“全心”的配合，定期的回診追踪檢查，這樣才是創造雙贏的不二法門，也唯有如此才能確保牙周補綴長期的使用。關鍵字:牙周補綴，牙周整形術，雙重冠覆蓋義齒，人工植牙

Abstract “Periodontal prosthesis is defined as those restorative and prosthetic endeavors that are absolutely essential in the treatment of advanced periodontal disease. Specifically, it refers to the treatment of the dentition mutilated by the ravages of periodontal disease; in general, its concepts, principles, and techniques may be employed in any restorative or tooth replacement service involving the nature dentition,” defined by Dr. Morton Amsterdam, DDS,ScD. There are significant differences between severe and mild periodontitis when making a treatment plan. Many factors need to be considered when periodontal compromized patients need periodontal, endodontic, orthodontic, and oral surgical dental implant treatments to obtain healthy periodontium. Controlling the etiology and improvements of periodontitis is the first priority before the long process of dental treatment comes to an end when dental prosthesis is finally delivered. Guided-tissue regeneration surgery can lead to new bone and soft tissue regeneration, therefore, allowing possible re-contouring. These techniques can achieve the goal that we all desire. Improving and maintaining healthy periodontal conditions and tissue will enable us to achieve the ideal prosthesis which will restore the functions and esthetics that patients desire and need. In this thesis we provide nine cases; three female and six male case studies ranging from ages 47 to 71. They all suffer from moderate to severe periodontitis. We provide patients with fixed crown & bridges, magnetic attachments, removable partial dentures, double crown over-dentures, traditional over-dentures, modified fixed bridges, and dental implant prosthesis for their perio-prosthesis through interdisciplinary treatment planning. So far, perio-prosthesis can satisfy patients with their needs for chewing, pronunciation, and esthetics. Yet these different designs need time to prove themselves which is why long term follow-up and maintenance is very essential. Perio-prosthesis needs thorough considerations from all aspects. Every patient’s treatment is based on their individual dental needs. We strive to help patients with their dental health needs and gain their confidence. This will help comfort the patients so they are willing to come back to our recall programs to create a win-win situation.
