糖尿病患者自我污名對自我照顧行為影響之研究

中文摘要本研究旨在探討糖尿病患者自我污名對自我照顧行為的影響。研究方法以立意抽取200位第2型糖尿病患者為研究對象，於高雄市4間糖尿病健康促進機構之門診進行問卷調查，並將蒐集之資料予以統計分析。研究結果發現：（一）在自我照顧行為項目中，以規律服藥行為得分較高，以血糖監測、飲食行為得分較低。（二）個人基本資料中，影響自我照顧行為的因素有罹病時間、病情告知情形、家族病史、糖尿病藥物治療方式。（三）自我污名與整體自我照顧行為、飲食行為、運動行為、血糖監測行為、足部照顧行為以及高、低血糖的預防與處理行為統計上呈顯著負相關。（四）經逐步迴歸分析後，影響自我照顧行為的預測因素為「自我污名」、「糖尿病藥物治療方式」、「病情告知情形」，其總解釋量為25.8％。最後，研究者對於糖尿病患者自我照顧行為執行程度、自我污名對自我照顧行為的負面影響、去除自我污名教育的重要性等部分提出討論，並形成建議。

Abstract The purpose of this study was to investigate the effects of diabetes’self- stigma on self-care behavior. Methods Study subjects were 200 patients with type 2diabetes in Kaohsiung,using a self-reported questionnaire, we evaluated diabetes self-care behaviors and the condition of self-stigma. Self-care behaviors were evaluated in six categories: diet, exercise, self-monitoring of blood glucose, medication taking,foot care,the events of hypoglycemic or hyperglycemic for prevent and appropriate treatment. The condition of self-stigma were evaluated using the self-stigma of diabetes scale. Multiple regression analyses were used to determine the association between self-care behaviors and self-stigma. Results and conclusions Results indicated that（1）the effect factors of diabetes self-care behaviors includes the time of diabete, the diabetes medicine treatment way ,the condition of disclosure,the family history of diabete.（2）The scores of self-stigma were significantly negative associated with diabetes’sef-care behaviors.（3）Self-stigma, the diabetes medicine treatment way, the condition of disclosure were the predictor of diabetes’self-care behaviors. The factors that influenced diabetes’self-care behaviors were discussed.
