以中醫頭皮針原理設計頭戴裝置應用於輕度認知障礙之研究

摘要


失智症(Dementia)是全世界共同發聲與共同面對的重要公共議題之一。而中醫頭皮針的穴位治療,是具有可以改善認知功能與延緩記憶力衰退之效果,如此有效之療程,往往因患者怕痛或是恐懼針灸帶來之疼痛感而無法落實此療法,而降低治療意願。 本論文以中醫頭皮針原理為基礎,設計一套系統包含頭部穴位按摩刺激與局部熱敷之頭戴式裝置,以及一部適合年紀較大的長者之八段錦運動指引影片,並以腦電波(Electroencephalography, EEG)之事件相關電位(Event-Related Potential, ERP) P300事件成分波之特徵量化為判斷認知功能程度之依據,並藉由此數據分析使用此系統前後之變化差異。 本論文在臨床前驗證招募24位受測者針對此頭戴式刺激裝置是否符合人因工程之設計,包括針對裝置舒適度,操作難易度等;以及在腦電波之事件相關電位試驗,招募了八位受測者在三週期間搭配本研究所設計試驗流程並搭配頭戴裝置以及八段錦指引動作影片,探討其腦電波之P300振幅及潛時變化並利用統計檢定方式探討其前後測之差異,希望此系統能協助臨床上別於制式化量表之方式,盡早發現並對輕度認知障礙之患者做治療及對於恐懼中醫頭皮針療法之患者有替代的選擇。

Dementia is a worldwide serious public health issue. Traditional Chinese Medical scalp acupuncture has the effect of improving cognitive function and delaying working memory degradation. However, such an effective treatment mostly cannot be implemented due to patients’ fear of pain for acupuncture, which would reduce the willingness for treatment. This study, based on the principle of Traditional Chinese Medical scalp acupuncture, designed a system with the function of head-wearing acupoints massage and local hot compress, and a Baduanjin exercise guide video for the elderly. Also, the study sees Electroencephalography (EEG) of the Event-Related Potential (ERP) P300 event component waves as the basis for the criteria of cognitive function. The experimental result can reveal the significant pretest-posttest difference. In this study, the 24 subjects were recruited to verify if the head-wearing device complies with the human factors engineering design or not. The experiment includes the comfort and difficulty of device operation, etc. As for ERP experiment, the eight subjects in it followed the procedures designed in this study, wore head-wearing device, with Baduanjin exercise and EEG signal recording/video for three weeks. In the data of the ERP experiment would observe the P300 amplitude, latency difference, and the significant pretest-posttest difference. This system is expected to develop the clinical methods differing from standardized scales, to do the treatment for patients with mild cognitive impairment as early as possible, and to offer alternative options for patients who are afraid of Traditional Chinese Medical scalp acupuncture.
