過去超音波針灸的研究多著重於症狀的治療效果,較缺少探討超音波操作模式的影響,導致發展超音波針灸系統上定規格與操作方式的困難。為此,我們以先前使用不同參數的超音波刺激合谷穴,所找出較佳的刺激時間、頻率、功率及工作週期等結果為基礎,進一步嘗試超音波刺激穴位來治療高血壓患者的臨床測試,以評估超音波針灸之療效。 本研究使用超音波刺激高血壓病患與健康正常人的足三里穴,超音波頻率為1 MHz與3 MHz、強度為61.7 mW/cm2 (ISATA)與125.1 mW/cm2 (ISATA)、工作週期為10%與100%,刺激時間為10分鐘。刺激前後受測者的經絡良導絡值、血壓與脈搏的變化量,都被量取以作為療效評估的依據。實驗中並安排假刺激組作為對照組,以探討心理因素的影響。 結果顯示,高血壓病患與健康正常人經超音波刺激足三里穴後,左右手六原穴的經絡良導絡值均呈現明顯下降變化(p<0.01),平均變化量最多可達刺激前的38%。由交叉比對結果發現,頻率3 MHz的刺激造成之平均變化量,較1 MHz的刺激多8%;強度125.1 mW/cm2的刺激造成之平均變化量,較61.7 mW/cm2的刺激多6%;工作週期100%的刺激造成之平均變化量,較10%的刺激多7%。此外,高血壓病患經不同參數超音波刺激之後,收縮壓、舒張壓與脈搏均呈現顯著的下降現象(p<0.05),平均變化量可達刺激前的3~7%。所以超音波刺激穴位確有降低高血壓患者之血壓與脈搏的療效。這些結果更確定超音波針灸儀未來應用於臨床醫療與研究的可行性。
Pervious studies on ultrasonic acupuncture were stressed on the therapeutic results of a certain syndrome and less works to explore factors affected by the ultrasonic operational modes. It therefore led to some difficulties on defining specifications and operation for the development of an ultrasound acupuncture system. To further assess effects of the ultrasonic acupuncture, we carried out clinical trials applying ultrasound stimulations on the Zu-San-Li acupoint to cure hypertension patients. The operational modes of ultrasound stimulations, including stimulation period, frequency, intensity, and duty cycle, were determined according to results of our previous preliminary studies. This study applied ultrasounds of 1 and 3 MHz, intensity of 61.7 mW/cm2 (ISATA) and 125.1 mW/cm2 (ISATA), and duty cycles of 10 and 100% to stimulate the Zu-San-Li acupoint for 10 minutes on both the hypertension patients and healthy subjects. The Ryodoraku values of the meridian, blood pressure, and pulses before and after ultrasound stimulations were measured to assess the therapeutic effects. For each experiment from a subject, result from a false stimulation was measured to be a control group for exploring results corresponding to psychological factors. Results demonstrated that after ultrasound stimulations on the Zu-San-Li acupoint, the Ryodoraku values of the six original acupoints of both hands measured from both the hypertension patients and healthy subjects were apparent decreasing (p<0.01). The average improvement was up to 38%. The following results were found by parametric study: the improvement by stimulation at 3 MHz is 8% better than that by stimulation at 1 MHz, the improvement with the intensity of 125.1 mW/cm2 is 6% better than that with the intensity of 61.7 mW/cm2, and the improvement with duty cycle 100% is 7% better than that with duty cycle 10%. In addition, the systolic pressure, diastolic pressure, and pulse of hypertension patients were obviously reduced (p<0.05) after the ultrasonic stimulation. The average improvement was about 3~7%. Therefore, it is evidenced that the ultrasonic stimulation on acupoint is effective on the treatment of reducing the blood pressure and pulse of hypertension patients. The feasibility of potential applications to clinical care and research is also demonstrated in this study.
