研發音頻電刺激系統

摘要


摘 要 在臨床醫療復健上,電刺激治療患部來達到鎮痛、舒緩神經肌肉、促進改善患部血液循環已漸成為主要復健療程之一。在目前電刺激治療過程中,除了達到降低患部疼痛感的目的外,刺激作用還會伴隨其他知覺感受器產生適應性的問題,這將會造成患者知覺感的暫時喪失,知覺感受能力的降低,此情況對患者來說是危險的。本論文目的,希望發展一套刺激系統來進行電刺激治療,利用音頻多變動頻率的刺激方式來改善知覺適應性的產生。 在論文中為檢驗以音頻刺激的方式來改善知覺適應的問題,設計一套電刺激對比實驗,經由38位受測者進行TENS、單頻刺激以及音頻刺激這三種刺激方式,從實驗數據中來探討這三種刺激對知覺適性的影響與刺激舒適程度的比較,實驗結果可推斷出以音頻刺激相較於另兩種刺激方式更能有效的改善知覺適應的情況發生,且在刺激舒適度的主觀感受上受測者接受度也有較佳的表現。 基於實驗結果音頻刺激能有效降低知覺適應的發生,利用相同原理設計一套以電腦為架構基礎的音頻刺激系統,系統整合應用PCI界面、單晶片(8051)控制以及MP3音頻壓縮等技術,來達成音頻刺激方式。藉由此系統,期能提供在學術上針對音頻刺激各參數能更深入的實驗設計及討論。在臨床應用上,能讓病患在不忽略刺激知覺感的情況下來進行電刺激能量的科學量化,避免電刺激意外的發生。在未來,更希望能建立電刺激療程處方資料,真正落實電刺激的科學量化。

Abstract The treatment of electrical-stimulus was an alternative method used in clinics to alleviate pain. It was also used to improve the blood circulation through kneading the local muscular groups. However, the electrical stimulation would fabricate the irritation sensation and would also induce sensory adaptation in the course of treatment. As result, the treatment of electrical stimulation would be dangerous for patients who has lost or who has deteriorated sensation due to the electrical stimulation. It is important to compensate this deficiency of using electrical stimulation. The objective of the study was to develop a stimulation system which would be utilizing dynamic audio frequency for making the conformal compensation to the sensory adaptation during the electrical stimulation. The self-developed electrical stimulation system was integrated using MCS-51 chip control and MEPG layer 3 (MP3) audio compression technologies. In order to assert the hypothesis that the electrical stimulation would prompt the sensory adaptation, a double blind experiment using three different stimulated methods such as bi-phase TENS, single frequency, and dynamic audio frequency were observed on 38 volunteers. The maximum strength of stimulus was set at 40V and 3 mil-A. The evaluation was on the awareness of stimulation and the undisturbed sensation upon the subjects’ subjective opinions. The results signified that using musical signal in the audio frequency stimulation would diminish sensory adaptation more than others. And, most subjects were poised by the stimulation of musical signal.
