The aim of this research is to design an in-house long term medication management system for chronic patients. Previous researches discovered that 63.5% of elderly people over 75 years old suffer chronic illness while only 11% of people under 65 years old have chronic illness. The reports also dedicated the 27% chronic patients take more than four kinds of medicines at home. Some surveys discovered that the main reason causing the old patients to admit to hospital repeatedly is not following the doctor’s instructions of taking medicine. In order to improve and supervise this situation this research designed an in-house long term medication management system for chronic patients. This medication management system includes a PC data input subsystem, a physiological parameters measurement subsystem, and an emergency alert subsystem. Patients enter the information including the frequency of medicine intake, timestamp and patients’ basic physiological parameters (blood pressure, ECG, and body temperature) through the PC data input subsystem. The PC will remind patient to take the medicine on time. The physiological parameters measurement subsystem then measures patients’ current physiological parameters and conveys the data to the PC. As the patient has not taken medicine yet or the physiological parameters are abnormal the PC will initiate the emergency mechanism and send message to alert the other family members immediately. The results of this research improve and supervise the problem that uses medicine, offer perfect medical management for home care application.
