中文摘要 雷射針灸是從傳統中醫學中發展出來模擬針刺的方法。現階段的雷射針灸設備大部分都還是體積龐大的醫療器材,對於可攜式的居家照護設備仍然缺乏,很多的疾病其實一般大眾可以藉由向這樣子的傳統醫學做治療。如此一來也為一般民眾節省時間,對醫療資源相對的也可以節省下一筆費用。 在此研究中將會以研製可攜式雷射針灸儀為主,在材料方面會使用低功率雷射二極體作為發射源,並利用單晶片控制治療波形、頻率及強度,其中在頻率上設計八段可調,分別為2、4、10、40、50、60、70、100Hz,參數上依文獻中常用之頻率做設計;儀器中提供三種波形輸出,包含弦波、方波、三角波;輸出能量依5mW為間隔,系統可以產生10、15、20、25、30mW之能量,在固定材料上,會使用電極片將雷射針灸達到可固定體表的目的。臨床上採用肥胖症作為研究觀察,與專業中醫師討論決定雷射刺激穴位,並且施作一個療程之觀察。 在臨床實驗的參數方面,參照臨床上使用過之參數作為使用標準。實驗的設定頻率在70(Hz),而在輸出能量定於30(mW)。使用光波長808nm之AlGaAs雷射二極體為刺激源。 實驗中分成三組,一為實驗組n=18;使用雷射針灸加上傳統中醫治療。二為對照組n=19;使用傳統中醫治療。三是控制組n=8;單純使用雷射針灸,實驗前各組間各項減重參數(BMI、WHR、體重、腰圍、臀圍)均無統計上的差異。在經過一個療程後,實驗組之效果明顯優於其他兩組。各項參數上有統計上的顯著差異(p<0.05)。 可攜式雷射針灸儀的研發,相信在擁有更多臨床的證實之後,會有越來越多的人了解到這樣子的方式會是一種不錯的居家治療方式。未來可加入更多與單晶片結合的功能,依不同病症設計療程融入儀器中,使一般大眾能更容易使用。
Abstract Laser acupuncture that was developed from theory of the traditions Chinese medicine was treated as a way to simulate needle. It can help to improve some of ailment. At present, most of laser acupuncture devices are still big and expensive, but some kind of portable laser devices which can be used for homecare are still seldom seen. Actually, lots of ailment can be improved by low level laser therapy and also easy to learn for the public. In this way, people can save the costs of time and medical resource. The aim of this study was to design a portable laser acupuncture device. The laser source was AlGaAs infrared laser diode with an emission wavelength of 808nm, and the microcontroller system was used as the control center of the apparatus. The adjustable parameters of the laser output were as follow: waveform, frequency and output power. There were eight kinds of frequency can be adjusted, including 2, 4, 10, 40, 50, 60, 70, 100 Hz. These parameters were chosen by the number of papers in the medical literature concerning the application of laser therapy; the device could change three kinds of waveform: sine wave, square wave and triangle wave; laser driver could drive 10, 15, 20, 25 and 30mW output power. This system used a self-adhering electrodes to fix laser diode on skin. The system was applid to climical case of obesity. The acupoints were chosen by a doctor of Chinese medicine. The experiment utilized the following parameters: frequency, 70Hz; strength, 30mW; wavelength, 808nm. Subjects were divided into three groups. In the experimental group (n=18) received a combination therapy: traditional Chinese medicinal therapy and laser acupuncture; the comparison group (n=19) used traditional Chinese therapy only; and the control group (n=8) just used laser acupuncture only. Before the treatment, there were no statistical significance among groups. After the treatment, experimental group had better effect than the other groups (p<0.05). People will understand that laser acupuncture is a good application in homecare after having more verification of climical case. In order to make it easier for everyone, laser acupuncture devices can be designed with microcontroller to combine treatments with functions in the future.
