氣壓式脈波量測系統之研製及應用

摘要


本研究的目的為建立一能即時量取血壓脈波之系統,此系統具有氣壓感測結合壓脈帶的高穩定性與操作方便等特性,而氣囊製作選用有拉伸彈力佳與抗老化等優點的矽橡膠為主要材料。此系統為三組感測器與氣囊能同步量取寸、關、尺三點之血壓脈波,改善先前文獻中所描述的缺點。 程式主要是以LabVIEW來撰寫,顯示介面並可儲存資料,而分析介面可用來量化波形轉折點U、P、V等座標,計算出脈波特徵參數(∠U, ∠P1, ∠P2, ∠P, t1, t2, t3及波形總面積)並提供給使用者分析。 臨床應用以睡眠品質優劣者之脈波為主,受測者經由匹茲堡睡眠品質量表把分成兩組,分別為7位睡眠品質佳(正常)者及12位睡眠品質差(不正常)者,透過氣壓式脈波量測系統擷取受測者寸口脈象,經分析介面找出特徵參數,再透過t-test統計分析找出睡眠品質佳及睡眠品質差者之間的差異性。 研究結果顯示,睡眠品質差者在脈波特徵參數會出現∠P1(除寸浮)、 ∠P2(除寸浮)及∠P明顯大於睡眠品質佳者,∠U明顯小於睡眠品質佳者, t1 除關脈外皆明顯大於睡眠品質佳者,波形面積在寸沉、寸浮及關脈明顯小於睡眠品質佳者。本研究亦發現氣囊製作的方法會影響實驗結果,未來可藉由精密的塑膠射出成型技術來改善氣囊的外在因素。 本研究研製三組可同時量測的氣壓式脈波量測系統,發現睡眠品質差者之脈相較之下會比較弦,血管阻力明顯增大,且於波形面積上發現寸關尺的臟腑存在著分侯學說,本研究的發現,相信會對中醫脈診的科學化有所助益。

The purpose of this study was to establish a real-time blood pressure wave measuring system. The system used an air pressure-sensing device which had three cuffs with high stability and user-friendly features. The airbag production was made of using silicone rubber as the main material. It had a good stretch and the advantage of anti-ageing. This system had three sets of the airbag and sensors. It could measure Cun, Guan, and Chi three parts of blood pressure pulse simultaneously, in order to improve the measuring techniques as described in the previously references. The display interface program was written mainly in LabVIEW. The program could record datum and the quantitatively turning point of the waveform, calculate pulse parameters(∠U, ∠P1, ∠P2, ∠P, t1, t2, t3, SUM) and make them available analysis for users. In the clinical applications, the sleep quality was tested nice or poor to compare with the pulse wave. Examinees were divided into 2 groups by Pittsburgh Sleep Quality Index (PSQI) that 7 adults with nice sleep quality and 12 adults with poor sleep quality. The pulse measurement system with pneumatic detecting method to capture radial sphygmogram, the pulse characteristic parameters could be determined by analysis interface. The t-test method was used to analyze the difference relationships between nice and poor sleep quality. The result of this research showed ∠P1(exclusive of cun-floating), ∠P2(exclusive of cun-floating), t1(exclusive of guan) and ∠P of the poor sleep quality adults were obviously larger than the nice sleep quality adults, but ∠U of the poor sleep quality adults were obviously smaller than the nice sleep quality adults. The curve area(SUM) of the poor sleep quality adults were obviously smaller than the nice sleep quality adults in the cun-deeping, cun-floating, and guan. Also, it was found that airbag production method could influence the measured wave results. For better measuring result, thermoplastic injection molding technology may be used to improve airbag external factors in the future. In this research, we developed the three sets of the pulse measurement system with pneumatic in the same time, found the poor sleep quality adults tauter than the nice sleep quality adults and made the vascular resistance increase. We also found that cun, guan, chi corresponded to different organs. The research achievement would be helpful for the scientific credentials of pulse diagnosis in Chinese medicine.
