建置符合DICOM標準之12導程ECG儲存系統

摘要


紀錄心電圖訊號以持續的評估病人的狀態,對於心臟病病患是非常重要的醫療依據。ECG紀錄通常因為各家廠商不同而有不同的格式,如何協調這些格式使之成為單一標準,對於醫院病歷而言是非常重要的。DICOM標準制定工作小組一在2000年提出了波形標準 (DICOM Supplement 30) ,該標準規範聲音波形、心電圖波形、血流信號及其他的影學影像紀錄。將ECG訊號以符合此規範儲存於DICOM系統中。對於解決單一標準ECG記錄格式是非常有價值的。 本系統將以JAVA語言實現將ECG訊號轉換成DICOM格式。本系統所產生的DICOM格式的ECG檔案同時也能被其他現有符合DICOM格式的程式讀取。未來可擴充直接從ECG放大器直接取得並轉換為DICOM格式或是將此系統建置為網路網路的ECG儲存及管理系統。

Recording ECG signal of the patient is a mandatory medical procedure to evaluate the condition of the patient continuously, especially for the patients who need further diagnosis and treatments. The records usually has been printed on a graph paper or stored in the proprietary format of the device’s manufacturer. How to harmonize the format and make a unified ECG record format is an important issue in the Hospital. DICOM Work Group 1 developed the Waveform Standard (DICOM Supplement 30) in 2000, which addressed the robust interchange of waveforms, including support for audio waveforms, ECG waveforms, hemodynamic signals and Holter in medical imaging context. Implementing this standard for ECG records will be highly valuable because the records can be integrated with other existed modality’s records that stored in DICOM format. An application which is written in Java language to generate ECG waveforms in DICOM format has been successfully developed. The initial result shows that the generated DICOM files which contains ECG waveform was successfully parsed, opened, and viewed with existing ECG applications. Further works still needed to extend the capability of the application such as to acquire the signal directly from the ECG device and to develop a web-based ECG viewer.
