根據文獻及衛生署統計資料顯示,乳癌實為現今婦女健康的頭號殺手,而乳房超音波及乳房攝影在乳癌診斷上仍有其限制。因此,國內外皆有進行乳房專用正子造影儀之研究,作為早期乳癌的診斷工具,以彌補現有檢測方法的不足。但正子乳房攝影儀的輸出檔案並非正式DICOM標準格式,故無法在異質醫療資訊系統間進行儲存、交換、管理和整合等動作。據此,本研究主要是藉由資訊與通訊科技建構支援正子乳房攝影之資訊系統,期望能改善此問題。 在本研究中,資料架構採用美國ACR-NEMA發佈之DICOM 3.0標準,將取得之影像檔及相關資料整合成符合DICOM標準格式之檔案後,以Image J影像處理工具進行初步的影像處理,並將處理前後資料放置MS SQL資料庫中與網頁連結,在網路傳輸過程中以SSL建立網路系統,使網頁具有https之安全功能,並具有使用者登入之功能,以提升使用者瀏覽資料的安全性,而在網頁介面上具備查詢的功能,方便使用者查詢與瀏覽。 藉由網路上蒐集之DICOM free viewer驗證20個系統封裝完成之DICOM檔案,在DICOM tags部份讀取正確率100%,而封裝完之影像與原始影像無差異;在網路傳輸上,利用傳輸前未經過加密之影像與傳輸後經過解密之影像相減,20張影像皆無差異,正確率100%。 本研究研發一套正子乳房攝影之資訊系統雛型,此雛型可將正子乳房攝影之影像資料DICOM標準化,並使這些檔案具備資料隱私與安全性,以提供解決異質資訊系統間資料檔案互通之障礙。
According to the reference and the statistics from Department of Health, it shows that breast cancer is indeed the number one killer of women's health now, and Breast Echo and mammography in breast cancer diagnosis are still limited. Therefore, there are many studies from domestic and foreign country in breast PET which is as a diagnostic tool for early breast cancer to make up the deficiencies of detection methods on hand. However, the output data of PET mammography is not the standard DICOM format, so the data was not able to be stored, exchanged, managed and integrated with different medical information systems. Hence, the purpose of this study was improving this problem by constructing information and communication technology to support PET mammography information system. In this study, the standard DICOM 3.0 which was announced by U.S. ACR-NEMA was used for the information structure. The image files and related data were integrated to be the standard DICOM format file, and then Image J image processing tools were used for preliminary image processing. After processing, information was input to MS SQL database to connect with the webpage. In the process of network transmission, the SSL was used to build a network system so that the webpage had the safety function of https and had the user login function to enhance browse data security for user. It also would be convenient for user to search and browse on the web interface. By collecting DICOM free viewer from the internet, we showed that the part of DICOM tags of 20 DICOM file system package achieved 100% reading accuracy, and there was no difference between the completed package of image and the original image; in network transmission, the non-encrypted image before transmission and the decrypted image after transmission were compared, the 20 images were no difference as 100% accuracy. Finally, this research developed a prototypical positron mammography information system to standardize the information of positron mammography as standard DICOM format. Furthermore, these files were capable of privacy and security in order to provide solutions for the barrier of exchanging information between different information systems.
