近年來,腎結石變成一種常見的泌尿系統疾病,指發生於腎盂、腎盞及腎孟與輸尿管連接部的結石,多見於30~60歲人群,且男性發病率高於女性,而近年來發病率有上升的趨勢。研究顯示,除泌尿系統感染、梗阻、自身代謝性疾病及遺傳因素外,腎結石的發病與不健康的飲食關係密切,先前研究常用單一中藥方劑治療腎結石其效果有限。 本研究的目的用兩個傳統中藥方劑豬苓湯及芍藥甘草湯做結合來探討治療腎結石之效應,本研究的第一步透過萃取分析與HPLC通過分析比對這些信號來判斷待測物所含有的物質不同化合物。第二步為製備單水草酸鈣結晶,然後以結晶學分光儀的研究方法,即結晶成核、晶體成長,用四種不同濃度猪苓湯抑制草酸鈣結石形成的抑制作用。第三步為動物實驗,以0.75%乙二醇餵食大白鼠形成腎臟草酸鈣結石作為模型,然後以四種不同濃度的猪苓湯合芍藥甘草湯以及0.75%乙二醇一同餵食三週期間。之後犧牲大白鼠和取腎切片,以顯微鏡觀察各個劑量在腎臟產生草酸鈣結石的多寡,做為評估療效的標準。 研究結果發現,以萃取技術和動物實驗的方法來探討,利用兩種方劑可以抑制草酸鈣結晶吸附在細胞表面,或是將已經吸附在細胞表面的結晶移除,而能將結石排出,故如本實驗方式能抑制草酸鈣結晶吸附細胞表面,四種不同濃度的猪苓湯萃取液,對結晶成核誘導時間有明顯延遲作用(p值<0.0001)及對結晶聚集的抑制百分比增加(p值<0.05),在大白鼠餵食乙二醇,誘導產生腎結石之實驗中,平均結晶沉積指數減少(p值<0.05)。動物實驗方面正常组表面光滑、柔軟,乙二醇组可見大鼠腎表面粗糙,並有許多白色晶體形成。豬苓湯及芍藥甘草组雖然表面粗糙,但是白色晶體較腎结石组明顯減少,藉由本研究的結晶成核的作用和抑制的機制跟大鼠的動物實驗可以充分的了解中草藥應用於治療腎結石的病症有更確切的療效。
The research aimed to use the Zhulingtang and Shaoyao Gancao Tang to reduce the formation of renal calculus. Initially, the analysis tended to be in accordance with the identification of object which contained different chemical compounds by the extraction and high pressure liquid chromatography. Second, the study applied calcium carbonate crystals which were extracted from waterweeds and the crystallography spectral instruments to trace the progress of the crystal formation of calcium carbonate in Zhulingtang. Animals with normal renal function were fed with or without three weeks of Zhulingtang and Shaoyao Gancao Tang in combination with 0.75% glycol because of establishing the model of the animal experiment. The concentrations of calcium oxalate were a standard subject using a microscope by anesthetizing mice and isolating renal section. In the results of animal experiment, two doses of extraction could prevent calcium oxalate from absorbing on the cellular membrane and excrete those crystals. Four levels of Zhulingtang extracted liquid suppressed crystal nucleation reaction (P<0.0001) and avoided crystal accumulation (P<0.05). The average of crystal accumulation decreased in association with the increase in renal stones by feeding glycerol to animals (P<0.05). It had been proposed that the kidneys of mice were soft and smooth in control group and those were rough and appeared with crystals in experimental group. This observation had been noted on experimental group adding Zhulingtang and Shaoyao Gancao Tang to decrease crystal formation. The study concluded that the Chinese herb could approach therapeutic goal of elimination of renal stones by the suppression of crystal nucleation reaction in our experiment.
