摘要

近年來國人對於醫療資源需求量逐年增加，如果無法有效利用醫療資源，會造成病患等待時間過長或醫院資源不堪負荷等問題。所以如何妥善利用醫療資源變成為一個重要的研究議題。個案醫院因各個院區電腦斷層掃描病患數量不同，導致有些院區電腦斷層掃描病患等候時間過長，影響電腦斷層掃描病患的醫療品質。有些院區電腦斷層掃描儀器使用率過低，無法有效利用醫療資源。所以，為解決個案醫院遭遇的困難，本研究利用醫院協同的概念，探討個案醫院中的三個院區，如何進行有效的協同合作，並利用協同的概念縮短電腦斷層掃描病患的等待時間，並提升電腦斷層掃描病患的醫療品質。
本研究利用系統模擬最佳化程序找出較佳電腦斷層掃描病患轉檢人數，並針對各種模擬情境做出決策與分析。例如：本研究對未來電腦斷層掃描病患人數增加提出解決之方法，並將此解決方法與個案醫院討論其可行性。最後，本研究探討個案醫院進行醫院協同的過程中，需進行哪些整合與改變，讓未來醫院協同的過程中可以更順利。

Recently, desires for medical resources have increased gradually. If hospitals cannot use medical resources efficiently, it will cause to the longer waiting time of patients or the shortage of the hospitals’ medical resources. Therefore, how to use medical resources more efficiently becomes an important research topic. For the case hospital, each branch has different number of computed tomography (CT) patients, so CT patients at the branch A have to wait for a longer time to do their CT scans. It will affect the quality of care of CT patients at the branch A. On the contrary, CT machine utilization at the branch C is low. It leads to waste the medical resources at the branch C. In order to solve the problem, this research adopts hospital collaboration concept to explore how to develop a patient referring mechanism among three branches through hospital collaboration. The objectives of this study are to shorten the waiting time of CT patients and enhance patients’ quality of care.
This research applies system simulation to optimize the patient referring mechanism and find the optimal solution of the number of referring patients among hospitals. Further, this study simulates different scenarios and analyzes their results. For example, this research designs a scenario that increases the current number of annual CT patients and simulates its optimal solution. The solution has been verified and discussed by the hospital’s manager. Finally, this research summarizes some suggestions of this case hospital collaboration. These suggestions can help implementation of future hospital collaboration.
