利用侵入式以及非侵入式之血壓波形評估動脈順應性

摘要


近年來由於血管硬化所直接或間接引起之心血管疾病患者日益增加,而臨床上侵入式血管攝影仍然作為評估血管硬化程度之主要方法,但與非侵入式相較之下其具有較高的風險性。本研究利用非侵入式血壓波形經由演算法運算其舒張末期後的脈波所得到之動脈順應性,探討與侵入式血壓波形其動脈順應性之間關係。本研究納入十五位受測者,結果顯示非侵入式與侵入式其衰減常數有良好的一致性(R= 1.3832)。 以前的研究表明衰減常數和在收縮期間的面積比所計算出來的動脈順應性具有良好的相關性。除此之外,結果發現使用侵入式脈波的衰減常數具有很高的相關性,因為其壓力脈波直接反應血管特徵。而在本研究中,結果表明使用非侵入式與侵入式的數據之間有良好的相關性,這可以假設本研究所提出的方法可以用於估算動脈順應性。

Recent studies have shown that patients who have atherosclerosis would increase the risk of cardiovascular disease. And, studies showed that arterial compliance could be used to assess the level of atherosclerosis. However, invasive method is still the primary method of assessing the atherosclerosis in clinical, because it has a higher accuracy for the atherosclerosis assessment yet with a higher risk. This study is to explore a methodology to assess arterial compliance with convenient measurement. The compliance will be derived from noninvasive blood pressure waveform was estimated by the decay constant from segment after diastolic period. There are 15 subjects were recruited to participate the study. The noninvasive pressure pulse wave decay constants were correlated with invasive pressure pulse wave decay constants with a good agreement. The decay constants estimated of invasive and noninvasive method has a good correlation of 1.3832.   In conclusion, previous studies have shown that the decay constant is well correlated with the arterial compliance calculated with the area ratio under systolic and diastolic phase. Apart from this, the results will find that using decay constants of invasive pulse wave will has a high correlation, because its pressure pulse wave directly reflects the vascular characteristics. And, In this study, we have shown that the noninvasive data has good correlation to invasive data. This could be assume that the proposed method can be used to estimate arterial compliance.
