台灣近十年來面臨人口老化與少子化的壓力,造成長期照護需求人數劇增,但又沒有良好的照護制度來照護年長者,使得青壯年將來所要負擔的經濟壓力增加,將會成為新的社會問題。目前台灣對於長期照護的社會價值、體制及相關政策,無法完全滿足被照護人及照護人的需求,根據先進國家長期照護的經驗,若要建置健全及良好的長照體制,除了政府政策規劃及立法推動外,必須規劃長期照護的經費來源及相關商業保險設計來補足政府所提供的長期服務的不足,同時更需要和專業團隊共同研擬及發展良好照護體系。而台灣過去所推動的社會保險把資源大量投入在「慢性醫療」和「長期護理」等相關問題上,和長期照護的真正需求脫節。因此本計劃將探討台灣長期照護商業保險與長期照護發展的關係,討論商業保險是否可以補足國民基本生存的條件,以改善政府長期照護所提供服務的不足,期望本研究結果在學術方面,能引發更多研究者的研究興趣與討論,而在實務面與政府政策上,能對長期照護與商業保險的發展走向做反思與檢討,並對台灣長期照護發展的困境提供一個解決方法。
Taiwan has been facing the pressure of aging population and lower birth rate in the last decade resulting in a sharp increase in the Long-Term Care (LTC) demand. However, a lack of adequate care system for the aged shall result in the economic pressure for the young and middle-aged generation which in turn becomes a new social issue. At present, the social value, system, and related policies for LTC in Taiwan can’t meet the demands of the cared and the caregivers. According to the LTC experience of advanced countries, in addition to policy planning and legislative promotion, it is imperative to plan the funding sources for LTC and related commercial insurance design to supplement the insufficiency of government long-term service to establish a sound and comprehensive LTC system. It is also necessary to collaborate with professional teams to design and develop a well-constructed LTC system. The social insurance promoted in Taiwan focused resources mainly in issues such as “chronic medical care” and “long-term nursing” which are out of line with the true demands of LTC. This project shall explore the relationship between commercial insurance in LTC and LTC development to discuss whether commercial insurance can make up the basic living needs of the citizens in Taiwan to improve the insufficiency of government services in LTC. It is expected that the project can academically inspire further research interests and discussions and bring reflection and review in practice and government policies on the development directions of LTC and commercial insurance and provide a solution to the dilemma of LTC in Taiwan.
