大規模塵爆燒燙傷治療程序之探討 -以八仙塵爆為例

摘要


鑒於過去對於塵爆事件大多只探討發生原因及預防,較少論及檢傷流程、治療滿意度、改善及成本分析,加上此次八仙塵爆,燒燙傷面積(TBSA) 40%以上的病患499人死亡12人,死亡率比歐美國家的類似案例要低,特別是燒燙傷達90%的死亡率只有美國一半。 為此本研究從國內外文獻歸納塵爆治療過程、有待需要改進之處及當現場有大量病患時,指揮、檢傷、後送三個重要部門需要緊密繫在一起,才能發揮最大的功用,因此須透過有效的指揮管理、檢傷分類方法、簡便的後送計畫才能有效率地將大量燒燙傷病患機制發揮最大功用,讓傷害降至最低,也希望透過本研究能讓下次燒燙傷事故特別是塵爆事件在處理過程上更進步、更有效率。

In view of the past, most of the dust explosion events only to explore the causes and prevention, less on the detection process, treatment satisfaction, improvement and cost analysis; Also in the case of Color Asia 2015, burn area (TBSA) more than 40% of the disease Suffering from 499 people killed 12 people, the mortality rate is lower than the similar cases in Europe and the United States, especially 90% of burns mortality rate is only half of the United States. For this reason, this study from the domestic and foreign literature to sum up the dust explosion treatment process, to be improved and when the scene has a large number of patients, the command, inspection, delivery of three important departments need to be considered together to be the greatest function , So through effective command management, inspection and classification methods, a simple post-delivery program can be efficient to burn a large number of burns to maximize the mechanism of injury, so that damage to a minimum, but also hope that through this study can make the next Burns accidents, especially dust explosion events in the process of progress more and more efficient.
