以個人數位助理為平台之短暫誘發耳聲傳射檢測儀

摘要


台灣新生兒聽力篩檢計畫早在1998年就已經開始推廣,但因為檢測儀器昂貴與自費的緣故,新生兒接受聽力篩檢的比率仍然偏低。本研究的目的是建構以個人數位助理為平台之短暫誘發耳聲傳射檢測儀。本系統在耳聲傳射擷取程序中,除了以衍生非線性響應(derived nonlinear response, DNLR)來消除刺激音,還使用平均的方式來降低雜訊。在聽力判定上的標準以再現性大於70 %且五個頻帶中至少三個頻帶通過為聽力正常。為了驗證本系統的功能,針對年齡約為22-79歲的6位正常聽力(Normal-Hearing, NH)和4位聽損(Hearing-Impaired, HI)的個案進行實測,實測結果與Otodynamics (Hatfield, Herts, UK)所生產的ILO292作資料比對,發現兩者對個案的聽力判定結果皆相同,並在時頻域圖上發現相似的頻率分佈與波形,由此證明本系統運用在聽力篩檢上是可行的。

Promotion of newborn hearing screening has been proposed since 1998. However, the low hearing screening percentage of the newborn remains because of expensive device and consequent high screening cost. The purpose of this study was to develop a PDA-based transient evoked otoacoustic emission (TEOAE) system. In addition to eliminate stimuli with derived nonlinear response (DNLR) method, the system decreased noise by averaging repeated acquired otoacoustic emission signals. Reproducibility over 70 % and three out of five passed frequency bands were used as criteria to determine if the hearing screening is passed or not. TEOAEs of six normal hearing and four hearing impaired subjects (22 - 79 yrs.) were measured to verify functions of our system. The measured results of our system were then compared with the screening results from commercial device ILO292 (Otodynamics Ltd, Hatfield, Herts, UK) and showed that both systems have the same screening results and similar wave forms and frequency distribution. These results indicated that the transient evoked otoacoustic emission system developed from this study could be used for hearing screening.
