醫療科技的進步使人們得以延長壽命,加上社會結構及家庭觀念改變,導致生育率降低,使得全球各國普遍邁入高齡化現象。然而,人口結構老化所帶來的問題中,又以老年人的安養照護問題最為迫切。而現今醫療環境競爭激烈,第一線的醫療工作人員的服務品質將成為醫院主要的競爭力,亦是增進與維持病人忠誠度的主要關鍵之一。 本研究以台灣中部某地區性醫院為例,首先藉由實地參訪瞭解院方之實務醫療照護服務方式,首先以居家巡迴照護服務模式為研究情境探討護理人員之資訊輔助需求,該模式主要是透過派遣護理人員至長期照護機構或一般居家,進行病人照護或管線更換等照護服務。然而目前院方仍使用人工方式進行派遣工作,相當費時耗力。 因此,本研究建置了一居家照護服務平台以輔助醫護人員進行巡迴居家照護服務,希望能提昇其服務品質。本研究並且運用基因演算法(Genetic Algorithm)來改善傳統人工巡迴照護排程之效率,並透過演算法取得每日照護服務的最短路徑來降低護理人員在巡迴照護服務上所花費的時間與成本。另一方面,本研究以顧客關係管理之概念為基礎,建置居家照護服務系統,協助護理人員於履行照護工作時,能夠隨時紀錄、存取與病人相關的資訊,藉由相關資訊系統來增進對於病患的個人化服務品質。
The progress of medical technology lengthen life of human being and the birth rate reduces by changing of social structure and family value, thus making global countries get into aging population. However, in problems takes by aging of population structure, the healthcare of elder is in instant to solve. Now a day the medical environment is competitive that the service quality of medical staffs of the first line becomes major competitive ability of hospital, also the key of increasing faith of patient. In this study, we first understanding that how actual medical care service work by observed and interview the hospital in Taiwan middle area. In the thesis, we discuss auxiliary information demands to medical staffs based on tour of home care services, the service is mainly sends nurses to Long-Term Care Facilities or patients' home, that provides services like patient care or pipeline replacement. Nevertheless, it is exhausted that the dispatching was done by hand. As the result above, we implement a home healthcare service platform to help nurses to do their tour of home care services and expect enhance the service quality. In the thesis, we apply the genetic algorithm to improve efficiency of scheduling tour by hand, also the algorithm can reduces time and cost in the medical tour that nurses spends by computing the shortest path of caring services every day. On the other hand, we implement the home caring system based on customer relationship management concept. As nurses do their medical caring work, they can record and access information about patient any time, and enhances the personalize services quality to patient by our information system.
