護理人員對長期照顧十年計劃2.0之認知及使用經驗-以台南市護理人員為例

摘要


為滿足長期照顧需求,因應政府長期照顧十年計劃2.0的推動,本研究採質性研究方式,深入訪談台南市護理人員對長期照顧十年計劃2.0之認知及使用經驗,逐步建構長照服務體系的問題與困境之意見。訪談對象為台南市執行長照相關業務的護理人員,訪談方式是直接與受訪者面對面答詢,訪談完後進行逐字稿編碼及資料分析,共有15位護理人員參與本研究。 研究結果顯示,受訪者普遍認為長照2.0是能幫助很多失能長者及身心障礙者提供照顧,很受益。但因各縣市管理機制不一,造成轉介和使用的困難。整合機制未建立,ABC層級的資源沒清楚的聯結,呈現混沌不明的狀態。資源分佈不均,仍有很多的長輩不知道長照資訊。再者,長期照顧由醫療照顧轉變為以生活照顧為主,應由各專業團隊來共同發展長期照顧體系。需其他專業團隊共同合作多元發展,使各專業在長期照顧工作上,都能有好的貢獻和發揮。長照2.0廣泛實行,能避免醫療浪費,讓個案獲得更良好、更完善之照顧。 最後強化長照人力及能力的問題,護理人員需熟知長照政策、加強專業能力、強化自身的價值;護理師的角色可扮演直接照護、個案管理、照顧管理、機構經營者等角色,以因應未來長照人才的多元需求。綜合前述護理人員的意見及相關文獻歸納後,本研究試著提出具體建議,以供未來推動長照政策時參考,同時可作為未來學術界長照政策執行研究的基礎資料。

As for matching the need of long-term caring and responding to the 10-year long-term care plan 2.0 promoted by government. This study used qualitative research method and had thoroughly interview with nursing staffs working in Tainan city of Taiwan for the correlation between their participation experience, cognition of the 10-year long-term care plan 2.0. Having better understanding of the experience of nursing staffs under 10-year long-term care plan 2.0 and difficulties and problems of organizing the long-term caring service system from scratch. There were 15 subjects in this study. All subjects were nursing staffs who practicing long-term caring work in Tainan city of Taiwan. The interview was conducted directly with subject individually and the transcripts were coded and analyzed. The result shows most subjects considered 10-year long-term care plan 2.0 is beneficial for incapacity elder and disabled. But due to the inconsistency of different system under different county, it is difficult for applying and transferring. The absent of integrated system and blurred connection among resources between ABC levels both contributed to this muddled situation. And unevenly distributed resources also led to lack of long-term care information among elderly population. Furthermore, the majority caring work in long-term care is daily-life caring, rather than medical care. Therefore, long-term care system should be developed by different professional group together. In order to elaborate different professions in long-term care work, each profession needs to cooperate with each other closely to bring better service to the client. The extensively implement of long-term care plan 2.0 could avoid the waste of medical resources and provide much more better and thorough care for client. Last but not the least, the issue of manpower for long-term care and their capability should be mentioned. Nursing staff need to be aware of long-term care policy, improve their professional capability and also strength their worthiness in long-term care field. Based on the response collected from this study and references reviewed, this study attempts to address specific suggestions for the promotion of long-term care policy in the future. And also laid the groundwork for other studies in scholar field referring to long-term care policy in the future.
