檢測退化性膝關節炎尿液試片之應用評估

Application evaluation of urine strips for Knee Osteoarthritis

摘要


足球是一項高接觸性、高衝擊性的運動,足球運動員膝關節的使用率遠比一般其他運動員為高,若膝關節受傷或使用過度造成膝關節軟骨磨損,將提早引發退化性關節炎甚至演變成早期關節退化。為預防退化性關節炎的侵害,早期發現、早期保養關節以延緩軟骨退化速度是非常重要的,幫助足球員延長運動生涯及減少醫療資源的浪費。本研究利用已開發之免疫層析試紙結合奈米金嫁接單株抗體來檢測尿液中的生物標記物以評估其作為退化性關節早期篩檢、療效追蹤之應用性。回歸統計結果P值小於0.001表示免疫層析尿液試紙結果與IKDC問卷總分間呈現顯著相關。60位非足球運動員IKDC問卷之無退化者平均分數為88±0.1分,有退化者之平均分數為49±0.4分;33位足球運動員中無關節軟骨退化者之IKDC問卷平均總分為87.2±0.2分,有退化者之平均分數為63.0±0.3分,顯示IKDC問卷總分對軟骨退化預測之有效性,而病患試片測試結果顯示診斷正確率達82%,顯示IKDC問卷結合COMP免疫層析尿液試片能夠成為退化性關節炎之檢測工具,研究結果也顯示對於足球運動員而言以WOMAC關節量表搭配免疫層析試紙評估退化性關節炎病患之玻尿酸治療效益是可行的。

Football is a kind of sports that are high-touch and high-impact. Once the football players get knee injuries or overuse their knees, it will lead to the joint degeneration even turn into an early osteoarthritis. To prevent the joint degeneration, take good care of knee cartilages and do the early detection are important. In this study, use the developed immunochromatographic strips that combine with nano-gold grafted monoclonal antibodies for detect biomarkers in human urine, and investigate the application of early detection for osteoarthritis and track the effect of the treatments. The logistic regression result shows the significant correlation between the urine strips and the IKDC scores. The IKDC average scores of non- degradation people are 88 ± 0.1and the scores of knee degradation people are 49 ± 0.4; the IKDC average scores of non- degradation football players are 87.2 ± 0.2 and the scores of knee degradation football players are 63.0 ± 0.3 points. The urine strips test results of osteoarthritis patients shows 82% diagnostic accuracy rate, indicating that immunochromatography urine strips can work with the IKDC scores and become the useful detection tool of knee cartilage degradation. Moreover, the WOMAC joint scale results and the urine test results of football player those who has hyaluronic acid treatment shows the potential of therapeutic efficacy evaluation.
