利用田口動態分析與自製塊規評量靜脈注射尿路攝影之X-ray影像品質的最小可分辨差異

摘要


本研究是利用田口動態分析方法,固態水假體及壓克力線群假體,評估成人靜脈注射尿路攝影的攝影參數與影像品質的最佳化,而成人靜脈注射對比劑的尿路攝影的影像品質因對比記得密度與結石相近,故要求更為嚴格,所以本研究因子的設定主要考慮了臨床常用參數,(A) 管電壓峰值(kVp);(B) 管電流與時間乘積(mAs);(C) 焦斑尺寸(Focal-Spot-Size);(D) 射源至影像接受器的距離(SID),等四項。每項因子各設立了兩個水準,並依照田口L8直交表的建議排列。成人腹部是以固態水假體堆疊而成(20cm、25cm、30cm),以壓克力線群假體作為影像解析度的分析標準。並將24 (8+8+8=24) 張影像結果經由三位臨床經驗豐富的放射師評分。經研究分析,動態最佳化的攝影參數設定為:64kVp、45 mAs、大焦斑、SID 87cm。於20cm;25cm;30cm之固態水假體可產生的影像最佳解析度為:0.76mm;0.88mm;0.96mm,相較於臨床習用參數所產生的影像解析度(1.11mm;1.62mm;1.18mm),靜態分析的最佳化攝影參數可提供穩定且品質良好的影像解析度。靜脈注射尿路攝影為臨床常用之泌尿系統結石診斷工具,本研究藉由田口動態分析方法,成功的分析出一組影像解析度良好且穩定的臨床X-ray攝影參數。期望能有助於臨床增加影像診斷價值,共同為患者之診療奉盡心力。

The optimization of intravenous urography was accomplished according to the Taguchi dynamic analysis in this study. In doing so, solid water phantom was assembled in three various thicknesses to simulate the adult abdomen in real X-ray exposure. The Taguchi L8 (27) orthogonal array was adopted to organize 4 factors with 2 levels into eight combination and still hold the precision and accuracy as it was organized in to full factor combination (2×2×2×2=16). The four factors were (A) kVp, (B) mAs, (C) focal spot size, and (D) SID; and the three different thicknesses of the solid water phantom was preset as 20, 25, and 30 cm, respectively. Every exposed X-ray image was scored by three well trained radiologists for three rounds under the double-blind principle, and the score criteria was given according to its contrast, sharpness, and spatial resolution as well. The obtained scores were reorganized according to Taguchi unique designation into fish-bone-plot to further analyze the contribution and cross interaction among factors for various thickness. The optimal combinations for intravenous urography under different thickness were (A) kVp: 66, 72, 64, (B) mAs: 45, 45, 45; (C) focal spot: small, small, large; (D) SID: 87, 87, 87 for 20, 25, and 30 cm thickness of the solid water phantom, respectively in this study. the three static analysis could integrate to explore the dynamic analysis and concluded that the optimal setting of intravenous urography was (A) 64 kVp, (B) 45 mAs, (C) large focal spot, and (D) 87 cm SID. The optimal imaging setting can provide stable and convincible images of the intravenous urography X-ray for adults with various abdomen thickness.
