護理人員對於臨床常用化學品安全的認知與態度之探討

護理人員工作職責為提供高品質的優良照護,在第一線執行醫療護理時,常為了搶救病患而忽略自身的安危,同時本身也可能遭受與職業有關的風險;因此,本研究目的在瞭解護理人員接觸化學物質時之相關情形,醫療執業場所與護理人員使用化學品的認知及態度是否有相關,並探討護理人員對於臨床常用化學品安全的認知與態度間相關性因素。使用描述探索性相關研究法,利用結構式問卷與方便取樣法收集資料,針對中部某科技大學護理臨床在職之護理研究所,與二技部學生為研究對象(n=147, Power>0.8),回收有效問卷147份,有效回收率87.5%,問卷內在一致性信度為「第四部份化學品全球調和制度分類標示認知」K-R 20=.804,「第五部分化學標示的重要性」Cronbach’s α=.824,Spearman-Brown Formula=.798,專家內容效度檢定CVI =.879。研究結果顯示,有無參加「臨床化學物質安全課程」與參加課程之護理人員對化學品認知有顯著差異,個人教育背景、工作年資、及本身獲得資訊來源則無影響;個人教育程度越高與相關在職教育越多,護理人員認為化學品重要性的態度面越正向;一般而言,護理人員對於化學品之標示認知之分數偏低,且護理人員對化學品的認知與態度,不因化學品的接觸次數多寡,及是否使用防護措施而有顯著差異。本研究建議學校及醫療職場雇主,增加護理人員臨床常用化學品的相關教育課程,並加強防護措施使用的宣導,保護護理人員可在安全的醫療環境中工作,以達成ICN所提出的「正向執業環境:優良職場=優質照護」。

There are many chemicals in daily nursing practice which can cause adverse effects on unprotected skin ranging from contact dermatitis to permeation of the skin and systemic toxic effects. Nurses often neglect the safety and risks on occupational exposure to chemicals while providing quality care to their clients. The purpose of this study was to investigate the knowledge and attitude of using chemicals among nursing staff. A descriptive correlational design with convenient sampling method was used. Questionnaires were distributed to on-job students who either study at graduate program or RN-BSN program at a university in Central Taiwan. The final sample included 147 participants and with a response rate of 87.5%. The knowledge of chemicals used in clinical setting significantly differed, depending on whether the nursing staff had participated at a course of clinical chemical safety or not. On the other hand, educational background, work experience and source of information made no differences in related knowledge among nurses. The higher education level and more related education received by the nurses, the more positive attitude of chemicals. In addition, results of this study revealed that nursing staff had poor knowledge levels on identifying hazard signs of chemicals. Either the experiences of contacting chemicals nor the use of protective clothing made impacts on the awareness and attitudes toward chemicals among nursing staff. Courses on chemicals safety should be included in nursing curriculum and in-service training course; uses of protective clothing should be emphasized, and chemical safety should be promoted. Thus,「Positive Practice Environments : Quality Workplaces=Quality Patient Care」, as declared by the International Council of Nurses (ICN) will be achieved.
