醫師人格特質及工作價值觀與醫療行為關聯性之研究

摘要


研究動機:醫師是醫院主要收入創造者,當面臨制度改變、經濟誘因不同時,會引發醫療行為改變。由於工作行為變異有5%~25%可用內外控人格變數來解釋,故本研究欲探討醫師在兼顧醫療倫理時,工作價值觀是否受到影響。研究目的:醫師人格特質、工作價值對醫療行為之影響,並進而探討工作價值觀對人格特質與醫療行為間是否具干擾效果。研究方法:針對醫學中心、區域、地區醫院醫師立意抽樣,有效問卷165 份,回收率82.5%。研究結果:醫師人格特質內控傾向為多;工作價值觀傾向內在價值;醫療行為較重視維護權益;醫師人格特質內控者,越重視工作價值觀。內在價值觀對人格特質值與維護權益行為有顯著的干擾作用,薪酬價值觀對人格特質與尊重自主行為達顯著干擾效果。結論與建議:醫師內控人格特質愈高,其醫療行為的倫理表現越低,越重視內在工作價值觀,表示醫師達成工作標準的企圖心強,然而醫師越重視工作價值觀,越容易忽略醫療倫理準則。另外,價值觀對人格與醫療行為有部份干擾作用,內在價值觀高的外控人格對維護權益行為高,薪酬價值觀低的內控人格對尊重自主行為高。最後,建議醫醫院管理者應強化醫師醫療倫理認知,維護病人權益,並依據醫師的人格特質及工作價值觀,擬定出最合適醫師的管理方針,達適才適任。

Purpose: The physicians are the main creator of income in the hospital. When the system and economic incentives change, it makes the medical behavior change. There are about 5% to 25% of the variability of work behavior will be explained by the in-control and ex-control personal characters, so the purpose of our study is confer if the values of work will be affected by the medical ethics values during physicians’ medical practice.Objective: The effective of physician personal traits and work values on the medical behavior. Methods:, Make purposive sampling to the physicians in medical center, regional, and district hospitals. The total effective questionnaires are 165 cases. The returns ratio was 82.5%. Results: Most of the physicians’ personal trait is trend to the in-control character. The value of work is to trend to the internal value, the behavior of medicinal is point to safeguard the right, the physician personality more internals, the more emphasis on work values. Values of the internal of personality traits have a significant moderating effect on the personality and behavior of protecting rights. The values of the salary pay have a significant moderating effect on the personality and behavior of autonomy. Conclusions and suggestions: The higher of the internal control character of the physicians, the less the ethical performance of their medical practice. Much more attention to reach the values of work, and much more ambition to achieve the standard of the work. However, the much more physicians attention to point on work values, the easier to overlook the code of medical ethics. In addition, the view on the values have some interference effect on the personality and medicinal practice, the higher intrinsic value of the ex-control personal characters will make more afford to protect their personal right, the low salary values of the in-control character are pay more attention to respect the autonomous behavior.Finally, we strongly suggest that the administrators of the hospital should to strengthen the cognition of the medical ethics to the physicians, and safeguard the patients’ right, and to propos a suitable and appropriate management policy according to the doctors’ personality traits and values of work.
