評估共用醫療資訊和醫療影像儲傳系統建置及健康資料庫設計之研究-以山地鄉為例

摘要

山地離島地理環境特殊，地廣人稀及交通不便，醫療資源缺乏，無法有效利用現有醫療制度與資源，其醫療服務資訊化程度卻明顯落後，共用醫療資訊系統技術的提升，及提供醫療品質改善機制。因此，若要能藉由HIS及PACS系統的功能，來發揮山地鄉共同照護網的功能，則有必要將當地衛生所、支援醫院及緊急醫療責任醫院(IDS)等三方，做跨區域的資訊連結。確保三方醫療資訊系統的基礎是彼此可用且標準一致，則是此醫療照護資訊整合系統的建置關鍵。
本研究開始以文獻探討分析為主，針對HIS、PACS和健康資料庫目前的架構與工作模式，整理出必要的執行項目及步驟，再與各專家學者進行座談，討論出具體的研究方向。本研究主要目標如下：
1、加強各山地鄉衛生所執行HIS和PACS系統的成效評估
2、藉由跨區域整合性HIS和PACS系統更新計畫，以對山地離島當地居民，規劃一完整門診的健康資料庫。
3、使HIS和PACS系統更滿足各山地鄉居民的需求
藉由三大目標的完成，本研究除了可以在未來30個山地鄉及18個離島地區，提供建置及執行HIS和PACS系統的參考標準外；同時，建構一完整的健康資料庫之後，可結合各項醫療資源，分享跨區域醫療的資源和團隊，避免醫療資源的浪費。

There are many aborigine reside distributing over in regions of mountains and off-shore islands. The composition of aborigine to the whole country population is less than 2%, but it’s obviously different in the state of physical health of residents in subject region of Taiwan. For example, the HLA and Mitochondrial DNA between these two groups are dissimilar Hence, it’s required for establishing a health databank specific for aborigine residing in the regions of mountains and off-shore islands. However, for the difficulty of communication among the dispersed neighboring areas and traffic, it provides a more convenient and higher quality of clinic service through the common health databank. And the difference between town and country will be decreased by the informationization of those regions. According to the conclusion of the plan of estimation and analysis of telemedicine efficiency in Taiwan from the Department of Health of Executive Yuan, there are some criteria for developing medical informatics system in the future, like it should be able to clinic and inspect transferring in common hospital informatics system, sharing electric records system and PACS.
In this proposal, we focus on the reviews analysis for the model of health databank and electrical circuit clinic. And we expect to integrate a concrete research report from essential factors and steps after discussing with relative experts.
The specific aims are:
1、Generation of a model that suiting for the health information system of health station and circuit clinic in regions of mountains and off-shore islands.
2、Design of a complete health databank specific for residents in regions of mountains and off-shore islands according to the update plans of integration of systems of PACS and common HIS.
3、Discussion of the application of digital medical images and electrical records for teaching and researching.
After achieving the four goals of this proposal, we can establish a health databank efficiently. And several valuable applications can be followed. For example, a system of medicine diagnosis development from screening this databank is very useful for doctors. Besides, researches can use the huge data for comparisons and analyses between aborigine and non-aborigine. It’s believed very helpful for relative drug design. In addition, combination and extension of each medical resource is very important for regional clinic teams sharing each other and avoiding waste
