長期照顧服務使用者連續性長期照護服務品質之研究

摘要


我國長期照護服務由三大體系同時提供服務,三大體系各有其服務對象、服務使用規定,較不易提供老年人的完整性及連續性全人照顧。為使老年人獲得全人完整性的連續性照顧服務,應該及早評估我國使用長期照顧服務是否已達完整性與連續性的全人照顧。本研究為瞭解老年人在其不同的疾病狀況、身體功能、家庭照顧資源,是否獲得適切的長期照護服務,以深入訪談已使用長期照顧服務的老人,在需要長期照護過程中是否獲得全人之連續性的服務。 本研究以使用中部地區醫院附設護理之家、獨立型護理之家、養護機構、日間照顧等長期照護服務之使用者為研究對象,採深入訪談方式收集資料,收集時間從2007 年1月至2007 年4 月止,共訪談15 位長期照護服務使用者。以質性研究的內容分析法進行資料整理與分析。 本研究回溯15 位個案,發現共有14 位未符合連續性長期照護服務品質。而在15 位個案中總共劃分為99 個階段,其中有52 個階段個案的長期照護服務可以滿足其需要,有47 個階段的服務無法滿足個案的需要。在47 個階段中,有13 個階段需要增加居家復健、有11 個段需要增加護理之家服務、有10 個階段需要增加居家環境改善、各有8 個階段需要增加居家服務及緊急通報系統、有4 個階段需要增加醫療照護、有3 個階段需要增加物理治療、有1 個階段需要增加日間照顧。 本研究發現有絕大部分的長期照護服務使用者,無法獲得連續性長期照護服務品質。而長期照顧服務使用者未能獲得長期照護服務原因,有不符合服務申請條件、個案獲得服務方便性不足、照護服務資訊不足、涉及個案及家人的隱私、服務時數無法滿足個案的需求等,使得長期照顧服務使用者有服務使用不足或是使用的服務不恰當的情形。所以透過本研究可以瞭解到長期照護服務使用者使用服務的問題,可提供日後改善長期照護服務體系的參考。

Long-term care services in Taiwan provided by three systems have their own service targets and regulations which are not easy to provide integral and continuity care for aged people. For a full and continuity care service for the elderly, we should evaluate whether the long-term care service has achieved comprehensive and continuing full service in Taiwan. The purpose of this study is to understand whether the aged people in different illness, physical condition and family care resources have received appropriate long-term care services.Through depth interviews with the elder people who use long-term care services, we examine whether they received comprehensive and continuing service during the process of long-term care. The research subjects of this study are the users of long-term care services in nursing home affiliated to a hospital, independent nursing home, care center, day care service in Taiwan. From January to April in 2007, data were collected through depth interviews with 15 users of long-term care services. Content analysis of qualitative research method was applied to organize and analyze the data. We recalled 15 cases and discovered that there are 14 of them do not meet the standard of continuing long-term care service quality. We divided into 99 stages for 15 cases among which the long-term care service can satisfy the needs in 52 stages while in 47 stages the needs are not satisfied. Among 47 stages, 13 stages need home rehabilitation service, 11 stages need nursing home service, 10 stages need home improvement, 8 stages need home care service, and 8 stages need emergency report system, 4 stages need medical care, 3 stages need physical therapy, 1 stage need day care. We also discovered that most long-term care service users cannot obtain continuing long-term care service quality for the reason including unqualified application; inconvenience of obtaining service, insufficient service information, privacy issues, individual or family preferences for 24 hours institutionalized long-term care service. There is inadequate use of the service or inappropriate use of the long-term care service. So, we tried to understand the service problems posed for the long-term care service users; and provided references for future improvement of the long-term care service system.
