救護技術人員參與急救訓練課程學習差異之分析研究

Analysis and Research on the Differences of the Emergency Medical Ttechnicians’ Learning from Participating in the Emergency Medical Training Course

摘要


救護技術人員是極具挑戰性的工作,除要有充沛的體力和足夠的膽識以因應隨時待命的各種機動性勤務外,豐富的急救知識和精湛的救護技術更是相輔相成缺一不可因素。因救護技術人員所面臨皆為瀕臨生死一瞬間的民眾,因此,衛生主管機關應明瞭救護技術人員之救護技能及影響急救訓練課程學習成效因素為何,並針對各項成績偏低項目加以輔導加強,以提升救護品質。 依據「行政院衛生署緊急醫療管理系統」資料庫登錄之救護技術員人數統計,截至民國98年5月底為止全國初級救護技術人員共計27,024 人。學員多來自民間救護車公司之救護技術人員、醫療院所救護車駕駛等。學員因基本條件不同,救護品質良莠不齊,再加上近年來民眾自我意識抬頭,殷切期盼救護品質提升有相對關係。國內相關研究除消防體系外,少有針對醫政管理領域急救訓練課程學習差異相關研究。 本研究係整合「行政院衛生署緊急醫療管理系統」資料庫和國內承辦初級救護技術員相關機構共622份學員名冊及成績資料進行分析。為使樣本資料具客觀性,本研究將學員受訓試卷資料套入行政院衛生署頒布發行之「救護技術人員管理辦法」規訂課程標準。所採用統計方法包括描述性統計、推論性統計分析、獨立樣本t檢定、單因子變異數分析。研究結果發現:(1) 學員性別對筆試和技術考成績並無顯著影響(2)訓練機構不同筆試及技術考分數無顯著差異(3) 受訓學員年齡並不影響受訓成績高低(4) 學歷對於受訓成績無影響。同時更發現應(5)建立標準化技術考規範及給分標準,方能使技術考成績更具客觀性(6)各模組課程時數,應建構合於「救護技術員管理辦法」課程標準規定之範圍,使授課內容能均衡發展。(7)職業道德觀應納入訓練課程內容以維護就醫民眾隱私和救護工作品質來將加強對初級救護技術人員之急救訓練課程學習。

Emergency Medical Technician is a job with real challenge. For responding every mobile duty during the standby moments, in addition to the abundant physical strength and the great courage as well as wisdom; rich knowledge and proficient skills in the first aid are the indispensable factors to the EMT.Because the EMT meets frequently the sick or injured persons approaching death, the official health authorities should know well the skills of the EMT and the factors influencing the EMT’s learning and achievements in the emergency medical training course. By focusing on the factors which carry lower achievements and find ways to improve them, the quality of the first aid and care will be enhanced. According to the database of the Emergency Medical Managing System set up by Department of Health, Executive Yuan, there are 27,024 elementary EMTs in the whole country up to the end of May, 2009.Most of them are the technicians of the civil ambulance companies and the ambulance drivers of the medical treatment organizations. Because the basic conditions of the technicians are different and the medical qualities are uneven, the self-awareness of the public is raising and it is expected eagerly to enhance the quality of the first aid and care to the sick and injured persons as confrontation. Except the fire control system, the domestic relative researches don’t concern much about the differences of the EMT’s learning from the emergency medical training course in the managing field of the medicine and policy. The research combines the database of the Emergency Medical Managing System set up by Department of Health, Executive Yuan, and the domestic relative organizations to be responsible for the elementary EMTs. 622 technicians and the data of their grades are used in the analysis. In order to make the data of the samples to be provided with objectivity, the results of the technicians’ tests in the training course were evaluated by the program standard indicated in the EMT Management Rules announced by Department of Health, Executive Yuan. The descriptive statistics, the inferential statistics & analysis, the independent sample identification, and the analysis of the singer factor’s variations are included in the research. The researching results show the followings: The first, the sex of the technician doesn’t have obvious influences on the paper tests and technical tests. The second, there’s no obvious difference on the results of the paper tests and the technical tests for the technicians trained in different organizations. The third, the ages of the technicians influence nothing to the grades of the training. The fourth, the educational background influences nothing to the grades of the training. The fifth, standardizing the regulation of the technical tests and the rules of the scoring will provide the technical tests with more objectivity. The sixth, the teaching hour of each module program should meet the requirements of the program standard indicated in the EMT Management Rules. Therefore, the contents of the course will be developed with balance. The seventh, the professional ethics should be covered into the training course for securing the patients’ private matters and the quality of the first aid and care.
