The presence of mesiodens may pose some clinical problems,especially in the stages of the primaryand early mixed dentitions.The most common clinical complications include central diastema,abnormal tooth eruption,and abnormal occlusal development.Mesiodens also contributes to bonedestruction,root resorption,pulpal pathosis,and/or the development of cysts.This article presents twocases of supernumerary teeth.The case 1 of twins had two inverted mesiodentes and rampant caries.Full mouth dental rehabilitation and surgical extraction of the mesiodentes were performed undergeneral anesthesia.The Case 2 had a retained #61 and an impacted mesiodens resulting indisplacement of #21.After surgical extraction of the mesiodens followed by orthodontic treatmentalignment,the esthetics and occlusion in the upper anterior region had been established.The timingfor surgical removal of the mesiodens is highly controversial.Early diagnosis and surgical interventionin the proper time may prevent the possible complications to the adjacent teeth and structures.(TaiwanJ Pediatr Dent 3(4):194-199,2003)
