Surgical and Orthodontic Intervention of Complicated Impacted Maxillary Incisors: Report of Two Cases
以手術與矯正治療複雜的上顎阻生門齒一二病例報告
摘要
上顎門牙異位萌出或上顎阻生牙是兒童牙科及齒顎矯正門診常見的問題。早成門牙組生的因素很多，其中贅生齒及牙根彎曲是兩個較為常見的局部因素。此外許多先天的遺傳疾病及營養失調也可能會引發或合併有多顆牙齒不能順利萌出的情形。本文報告了兩個較為少見的病例。第一個病例是一位八歲六個月的男孩，他的左側正中門牙是上下顛倒的阻生牙，同時合併有未萌出的贅生齒及未脫落的乳牙門牙。第二個病例是一位十一歲六個月的女孩，他的左上顎正中門牙因為牙根大角度彎曲，造成水平阻生，同時合併有空間喪失，中線不正，嚴重空間不足及雙側下顎側門牙先天缺牙等問題。本文詳述如何利用手術及矯正牽引的方法來引導患牙到正常的咬合位置。

It is not uncommon to see an impacted or unerupted maxillary incisor in the children's dental clinic. The etiologies of impacted maxillary incisors are multifactorial. Among them, supernumerary teeth and root dilacerations are two common local factors. In addition, genetic factors and nutritional imbalance can also affect the tooth development and eruption. This report presents two complicated cases of uneruption. These are 1) the case of an inversely impacted permanent maxillary incisor secondary to a supernumerary tooth, and 2) the case of severe root dilacerations of a horizontally impacted incisor with midline deviation, severe space deficiency and congenital absence of bilateral mandibular lateral incisors. The purpose of this article was to illustrate the surgical technique and orthodontic traction bringing the impacted teeth into proper alignment.
