Common Musculoskeletal Disorders in the Hemophilic Patients

摘要

自從民國73年7月18日台大醫院成立國內第一個血友病醫療中心以來，至今已有150個血友病病人接受過物理治療專業人員詳細檢查他們的肌肉骨骼系統，本文回顧國外文獻，配合過去的經驗，描述血友病中心病人在膝、肘、踝、肩、髖等關節及其附近組識常見的問題，並列出台大血友病中心的病人這些問題的發生率，且探討這些問題發生的原因、機轉，與其對病人功能與日常生活的影響，此外對肌肉或骨骼系統出血後之處理與治療原則，作較詳細的描述，並對運動治療的方法與原則作更進一步說明，以供國內物理治療從業人員參考。

The first hemophilia center in our country has been set up in National Taiwan University Hospital since July 18, 1984. Till now, one hundred and fifty hemophilia patients had been evaluated by physical therapist. According to our evaluation and literature review, the common pictures causes, and pathologic processes of musculokeletal disorders about knees, elbows, ankles, shoulders, hips and their surrounding muscles and tissues of the hemophilic patients had been described. The incidence of those disorders and their influence on our patient's postures, gait patterns, ambulatory function, squatting ability and activities of daily living had also been reported. In this article, the essential treatment principles for the early management of hemophilic bleeding and ordinary rules for the management of hemophilic arthropathy were discussed. The principles of therapeutic exercise were especially emphasized. All the information mentioned here would be the guidance for physical therapists in the evaluation and mangement of hemophilic patients.
