The pectoralis major myocutaneous flap (PMMCF) is a convenient tool for the reconstruction of maxillofacial soft tissue defect after tumor extirpation. For the repair of large thru-and-thru facial defect, however, folding of this flap for intraoral lining is sometimes performed and may predispose to an umpredictable prognosis due to the compromised circulation distal to the folded area. Another choice for the intraoral repair is the tongue flap which may complicated with the disturbance of its postoperative function. In the case report, a case with the large facial defect repaired by the combination of the PMMCF and trapezius flap is proposed. Post-operatively, the esthetics is satisfactory.
