三軍總醫院顏面骨折病例之分析

中顏面骨與下顎骨是構成顏面主要的部份。由於工商發達，人口增多，交通工具的便利，人與人間來往頻繁,活動空間變大，相對地，造成顏面部外傷機會增加。本文主要目的是探討三軍總醫院由1992.7.1起到2000.6.30這八年內顏面部骨折1075個病例加以統計分析，並就性別，年齡層、骨折部位，造成原因、治療方式作一討論；另外再與本院1982年顏面骨折分析的結果，相互比較，看是否有異同之處。結果顯示：(1)男女性比例約為3:1，而在1982年男女之比是9:1。(2)骨折發生率以16-30歲的年輕人比例最高。(3)中顏面骨折以顴骨最多，下顎骨骨折則以下顎骨聯合處最多。(4)造成骨折最主要原因為交通事故，以機車事件最高(50.5%)，次為汽車事件(22.9%)。(5)治療方式則依咬合功能是否異常，臉型是否不對稱及骨折移位大小等因素，來決定採用開放式或封閉式骨復位手術。此結果再與十年前之統計來比較，除了人數增加外，女性患者比例顯著提高；而機、汽車事故仍是造成顏面骨折的最主要原因。由此臨床統計分析的結果來看，機、汽車在顏面外傷事件中，扮演很重要的角色。如何能降低機、汽車事故是社會大眾不可忽視的課題。

Maxilla and mandible are the major parts of the maxillofacial structure. Due to the prosperous development of the industry and the business, convenience of communicating transportation, increasing social contact and the extending scope of action, however, incidence rate of facial trauma is also increasing. The purpose of this investigation is to explore and analyze the 1075 facial bone fracture cases, collected within 8 years from 1992-7-1 to 2000-6-30, in respect to their sex, age fracture sites and the causes. In addition, the results were compared to the results of the 11982 to reveal if there is any difference. The results of the investigation can be summarized as follows: 1) Male: female ratio is about 3:1 whereas the ratio was 9:1 in 1982. 2) Most victims were with age from 16 to 30. 3) Most fracture sites were on zygoma in maxilla and symphysis in mandible. 4) Major case for the fractures was the traffic accidents, among which motorcycle accident was dominant (50.5%) and was followed by the car accident (22.9%). 5) Treatments were either open reduction or close reduction, based upon the following factors, such as whether there was malocclusion, facial asymmetry, and the amount of fracture bone displacement. In comparison to the results of ten years ago, it was shown that the victims was increasing in number, and so as the increasing female proportion in the victim population. Motorcycle accident and car accident still were the major causes and played very important roles in the facial bone fracture. Reduction of the motor accident occurrence should be the major concern for this society.
